Description
This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation is being issued as Request for Proposal #565-02-02; award will be made using the simplified acquisition procedures. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-23. This acquisition is unrestricted. The North American Industry Classification System (NAICS) 621511. REQUIREMENT: The VA Medical Center, Fayetteville, N.C. intends to solicit proposals and award a firm fixed price contract under the authority of Title 38 USC Section 8153, pending availability of funds for diagnostic studies. Services will be provided to patients of the VA Medical Center, Fayetteville, N.C. off site at the contractor's place of business. The contract is for one (1) year with four (4) one-(1)year option periods to extend. Contractor will provide non-invasive vascular lab services to include arterior lower and venous lower studies, and bone density measurement test (DEXA). Service will include the use of equipment, facility, nursing , technical and professional personnel to perform the specified studies. Patients will be scheduled for routine procedures by the scheduling department of the VAMC during normal business hours. If emergency services are needed after hours, arrangements will be scheduled by the Medical Admin Duty Officer of the Day (ADO). Both inpatients and outpatients will be scheduled for studies. Appointments for inpatients will be scheduled as soon as possible but no later than two (2) days after receipt of request. Appointments for outpatients will be scheduled as soon as possible but no later than two (2) weeks after receipt of request. Emergency services must be provided as indicated by the nature of the emergency but no later than two (2) hours after notification of an emergency. The VA will be responsible for coordinating arrangements for patient attendant, transportation and necessary medical records of patients to and from the contractor's facility. A final report of the exam and findings will be provided to the VA Medical Center Administration Service for filing in the patient's chart within 24 hours of the study. Reports are to be delivered to MAS (136), VA Medical Center, 2300 Ramsey Street, Fayetteville, NC. Any critical findings from the study requiring immediate attention requires physician to physician telephonic contact. The following information is provided as general information. Work hours of the VA: Monday through Friday -- 8:00 a.m. -- 4:30 p.m. National Holiday: The ten holidays observed by the Federal Government, i.e. New Year's Day, Martin Luther King's Birthday, Washington's Birthday, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving, and Christmas Day and any other day specifically declared by the President of the United States to be a national holiday. Nonworking hours (normal on-call hours) Monday through Friday, 4:30 p.m. to 8:00 a.m., and all Saturdays and Sundays. Emergency On-Call Hours: 12:00 Midnight Sunday through 12:00 Midnight the following Sunday. The contractor shall be responsible for protecting the personnel furnishing services under this contract. To carry out this responsibility, the contractor shall provide the following for their personnel: worker's compensation, professional liability insurance, health examinations, and income tax withholding and social security payments. The parties agree that such personnel shall not be considered a VA employee for any purpose and shall be considered employees of the contractor. Payment for any leave, including sick leave, holiday or vacation time is the responsibility of the contractor. The following provisions apply to this solicitation: 52.212-1, Instructions to Offerors-Commercial, 52.212-4, Contract Terms and Conditions-Commercial and addendum 52.237-3 Continuity of Services, 52,237-7 Indemnification and Medical Liability Insurance, 5.217-5, Evaluation of Options, 52.217-8 Option to Extend Services, 52.217-9, Option to Extend the Term of the Contract, 52.224-1, Privacy Act Notification, 852.270-4 Commercial Advertising, 852.271-70, Services provided beneficiaries, 52.212-5, Contract Terms and Conditions required to Implement Statutes or Executive Orders-Commercial, including clauses incorporated by reference under Section b (1)(5)(11-16)(23-24); Quotes must be in writing and include an all inclusive cost per test for the following tests: Arterial lower studies CPT Codes 93923 & CPT Code 93925, estimated number of tests per year is 60. Venous Lower Studies CPT Code 93965 & CPT Code 93970, estimated number of test per year is 12. Cost per Test for Bone Density Measurement Test (DEXA) to include two views (hip & spine) and interpretation, plus a lumbar x-ray on women over age of 55. Estimated 6 per year $. Cost per test will be bill at the current Medicare allowable reimbursement rate applicable and related services for North Carolina. Contractor will provide a breakdown of items by CPT codes, descriptions and current Medicare allowable rates effective at time of award. Option years will be awarded based on the current Medicare allowable rates in effect at time of notification of extension. Award will be made to the offeror who quote conforms to the requirements of the solicitation, best meets the minimum needs of the Government and has the lowest price, all factors being considered. Award may be made without discussions. The information required in provisions 52.212-1, 52.212-2, a completed copy of provisions 52.212-3, Offeror Representations and Certifications, Commercial Items ,852.219-70, Veteran Owned Small Business certification must be submitted with proposal. Technical proposal to include the following evaluation factors: company experience, past performance compliance with solicitation requirements and personnel qualifications. Contractor will provide the following supporting information to facilitate evaluation of the proposal: brief resume of company, proof of licenses, certificates, copy of indemnification and medical liability insurance, etc. references with points of contacts. Technical and past performance, when combined, are approximately equal to cost or price. Award will be made to the offeror who offer is the best value to the Government, price and other factors considered.
