Description

This is a notice of the intended issuance of Request for Proposal (RFQ) 663-13-02, to Provide Adult Foster Care Services to the beneficiaries of the VA Puget Sound Healthcare System, 1660 So. Columbian Way, Seattle, Washington. The RFP is in a commercial format and the package will be available on or about September 11, 2001. Multiple Award Indefinite Delivery Contracts will be issued according to requirements within the defined geographical areas. Delivery orders under the awarded contracts can be expected following contract award depending on the number of awards made. As part of the Veterans Healthcare Administration Adult Foster Care Pilot Program, the VA Puget Sound Healthcare System is seeking vendors/contractors to provide contracted Adult Foster Care Services to eligible veterans in the following Washington counties: Chelan, Clallam, Grays Harbor, Island, Jefferson, King, Kitsap, Kittitas, Lewis, Mason, Pierce, San Juan, Skagit, Snohomish, Thurston and Whatcom. Applicants must be those who are licensed providers of Assisted Living Services in facilities such as adult Family Homes, Adult Foster Homes, Residential Care Facilities and Assisted Living Facilities All facilities wishing to participate in this program must accept Medicaid. There will be two (2) meetings for vendor applicants wishing to participate. These meetings are crucial to the ultimate success of this pilot program. The meetings will be held on September 25, 2001, at 1:00pm in Seattle and September 27, 2001, at 1:00pm in Tacoma. The Seattle location is at the VA Puget Sound Healthcare System, 1660 So. Columbian Way, Bldg. 18, Room 111. The Tacoma location is VA Puget Sound Healthcare System, American Lake, Bldg. 17, Room 103, Tacoma, Washington. Questions about the Adult Foster Care Pilot Program and the contracting process will be addressed at that time. The solicitation will be available for download at no charge on or about September 11, 2001, via the VA Business Opportunities System website at http://www.va.gov/oa&mm/busopps/sols.htm. In accordance with FAR 5.102(a)(7), solicitation dissemination is limited to electronic medium, and no hard copies will be provided by the VA. Any amendments to the solicitation will be posted at the web site. Offerors are advised that they are responsible for obtaining all solicitation documents and amendments. The due date for receipt of proposals is on or about October 11, 2001. Electronic offerors will not be accepted. The North American Industry Classification System Code for this solicitation is 623311, size standard is 10 mil.
