- Description
- Scope of Work: This is a combined synopsis/solicitation for commercial items prepared in accordance with subparts in the Federal Acquisition Regulation 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-27. The Government intends to procure the Leadpoint, Micro Electrode Recording System, manufactured by Medtronic, on a brand name basis. The NAICS code is 421450. The system features the following characters: 1)The system must provide clean recordings of individual cells in the brain while filtering out electrical interference from normal operating room equipment. The system is used to help us confirm the location of brain regions that we have targeted for high-frequency deep brain stimulation to treat veterans with advanced, medication-resistant tremor and Parkinson's disease; 2)The electrode-handling portions must be compatible with Leksell arc, Model G, which is eight years old and is a "lateral mount only". The components of the system that connect to the arc must have a very small x-y footprint; 3) The system must be FDA-approved; 4) On-line signal display and advanced audio feedback of recorded signals, simultaneous multi channel recordings, recording and logging of signal sequences for later review, reporting and documentation, pre-defined editable notes/free text notation, making systematic documentation fast and efficient, and user defined windows and screen layout. Offeror shall propose pricing for the Micro Electrode Recording System which consists of the following components: (1)ea 2 channel Micro Electrode Recording unit, including: Patient unit, Leadpoint Front-End PC-Card and 33B29 earth cable; Leadpoint Software; (1)ea Notebook PC (minimum specifications) including w/windows 98,keyboard, 650MHZ Intel Pentium III CPU w/128MB RAM, 11.2GB Hard Disk Drive, CD-ROM Drive 48X speed, 3.5" Floppy Disk Drive, 14.1" TFT display (1024X768 internal & 1280X1024 external resolution), 16 bit Sound Blaster Pro compatible, External mouse; (1)ea External speakers; (10)ea Single use MicroElectrode Cable (start up kit), Sterilized 3-M active shielded cable for microelectrodes, DIN to 2*0.8 mm and one alligator clip, including a clip for fixation of cable; (1)ea Laser Printer, Black and White; 2 days on site training; one-year warrantay, and one-year warranty to include parts and labor. In addition, offeror shall propose prices for the listed below Hardware and Srvice Options: (2)Micro Targeting Drive System w/Sterilization case; (2)Stepper Motor Drive Option (Includes 1box of drapes); (1)Letsell Stereotactic Adaptor; (1)Radionics Stereotactic Adaptor; (2)Array Electrode Insertion Tube Set w/Carrier; (1)Micro Targeting Electrode 291A (bipolar 5 electrode array set, single use) length calibrated for Micro Targeting Drive System with Adaptor; (2)Electrode Sterilization Tray; (1)Knurr Trolley; (1)Carrying Case "WR" version. The Contractor warrants that at the time of delivery, the system will be free from defect in material and workmanship and will be in good working condition. Delivery of the equipment shall be FOB Destination to: Warehouse, VA Greater Los Angeles Healthcare System, 11301 Wilshire Blvd, Los Angeles, CA 90073, the equpment will be used by the department located at Bldg 500, RM 3051. The required date of delivery is 14 calendar days from receipt of award. Contrator will be responsible for the installation of system. Offeror shall submit a completed copy of representations and certifications at FAR 52.212-3, Offeror Representations and Certification-Commercial Items (May 2001).
