Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in the notice. This announcement constitutes the only solicitation: bids are being requested and a written solicitation will be issued only on request. Solicitation number DTFH6801Q000349 is being issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-27. Provide & Place a Slurry Seal on the main entrance road of the Tishimingo NWR. This slurry seal shall meet all requirements of the International Slurry Surfacing Association's (ISSA)(available upon request) "Performance Guidelines for Emulsified Asphalt Slurry Seal -- A105" -- Type II Slurry Seal. The section to be surfaced is approximately 7700 feet(long) x 24 feet(full width) on the main entrance road of the Tishimingo NWR near Tishimingo, OK. The slurry seal will begin approximately 50 feet north of the main entrance gate and proceed south to the intersection of Goosepen Rd/Sandy Creek Road. The slurry seal shall meet all specifications detailed in the ISSA A105 Guideline. NOTE -- All Flagging, Traffic Control Device and Permanent Pavement Marking items will be considered subsidiary to the slurry seal and should be included in the price quoted. Provide flagging for traffic control during placement, initial cure and temporary lane closures of the slurry seal operation. Provide traffic control devices (per Part VI of the MUTCD) for lane closure/delineation during placement & cure of the slurry seal. One-way traffic shall be maintained at all times. Provide & place permanent traffic striping paint over finished slurry seal. Striping layout shall match existing striping currently in place. (Solid 4 inches white edge lines {both sides of roadway}, and one, 4 inches-double yellow centerline stripe). Estimate of Quantity Slurry Seal, ISSA Type II: 185,000 s.f. Completion Date: November 2, 2001. MATERIAL SOURCES: The contractor is responsible for these sources, including established commercial sources. Use sources that fulfill the contract quantity and quality requirements. Determine the quantity and types of equipment and work necessary to select and produce acceptable material. Secure all permits and clearances for use of the source and provide copies of the documents. Do not use materials from a source that is unacceptable to the Government. Dispose of unacceptable materials and locate another source at no cost to the Government. PAYMENT: The accepted quantities, measured as provided above, will be paid at the contract price per unit of measurement for the pay items listed. Payment will be full compensation for the work prescribed in Construction Requirements above. PAYMENT WILL BE MADE UNDER: Pay Item: Asphalt Slurry Seal -- A105" -- Type II Slurry Seal. Approximately 7700'(long) x 24'(full width) on the main entrance road, flagging, traffic control devices and striping. REQUEST FOR QUOTE: Provide a quote for completion of project. The provisions at 52.212-1, Instruction of Offerors Commerical, applies to the acquisition. The provision at 52.212-2 Evaluation Commerical Items applies to the acquisition. EVALUATION FACTORS: The evaluation criteria are price. Bidders are to provide a list of 3 references. Bidders are to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications Commercial Items, with its quote. The Clause at 52.212-4, Contract Terms and Conditions Commerical Items, applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items, applies to this acquisition, of which the following are applicable: 52.222-21, Prohibition of Segregated Facilities; 52.222-26, Equal Opportunity; 52.222-35, Affirmative Action for Disabled Veterans and Veterans of the Vietnam Era; 52.222-36, Affirmative Action for Workers with Disabilities; 52.225-13, Restriction on Certain Foreign Purchases, 52.232-34, Payment by Electronic Funds Transfer Other Than Central Contractor Registration. BID SUBMISSION REQUIREMENTS: Bidders are required to submit a total amount for the completion of the project, based on the above described contract terms and conditions. 3 performance references, and the Offeror Representations and Certifications Commerical Items, at 562.212-3.
