- Description
- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation 541-011-02 is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-27. This is a competitive, 100% Small Business Set-Aside, for commercial items procurement under the North American Industry Classification System (NAICS) Code 561720 and Small Business Size Standard of $12.0 Million per annum. The Louis Stokes Cleveland Department of Veterans Affairs Medical Center (LSCDVAMC), Cleveland, Ohio is contracting for all labor, material, equipment, transportation and supervision necessary to perform outside cleaning of ductwork, overhead pipes, registers and grill work, in the Laundry Plant, Bldg. 46, of the LSCDVAMC, 10000 Brecksville Road, Brecksville, OH 44141. Services shall be for the base year period of October 1, 2001 through September 30, 2002, with four one-year pre-priced options for renewal. The contractor provided price schedule shall contain four (4) cleanings per year, and shall include pricing for each option year renewal. Contractor shall be required to perform one (1) cleaning per quarter, for a total of four cleanings per year, as prescribed herein. Each cleaning episode shall be completed within the first five days of the following months; October, January, April, and July. Cleaning service shall be in accordance with standard industry practice, using industry standard materials, equipment and procedures. The size of the building required for cleaning is approximately 25,000 square feet. Contractor shall cover all laundry plant material, supplies, furniture and equipment using a protective type covering so as to ensure said items remain free of lint, dust and debris. Contractor shall sweep floor clean of all lint, dust and debris, and provide proper disposal of said debris. The hours of service shall be 3:30 p.m. to 12:00 p.m. Monday through Thursday, excluding Federal holidays, or contractor may coordinate a Saturday cleaning from 7:30 a.m. through 3:30 p.m. Each cleaning episode shall be completed within the eight-hour periods established herein. Contractor shall contact the Laundry Plant Manager at least one week in advance of the date he/she plans to start each cleaning. The Laundry Plant Manager may be contacted at (440) 526-3030 extension 6290. The Laundry Plant Manager will inspect the cleaning service at the time of performance. Should the Laundry Plant Manager determine that the area requires re-cleaning contractor shall re-clean areas as instructed at no additional cost to the Government. For failure to perform cleaning services within the established performance period, a deduction of 1/90th of the quarterly rate shall be taken for each day the area remains unclean. Furthermore, failure to perform any of the services as set forth in this contract may be considered grounds for termination for cause under the termination for cause clause of the contract. Payment will be made quarterly, thirty (30) days in arrears, upon receipt of a proper invoice. Invoices shall be submitted in accordance with Federal Acquisition Regulation 52.212-4(g) and shall be submitted to, Chief, Fiscal Service 04(B), VA Medical Center, 10000 Brecksville Road, Brecksville, OH 44141. The provisions at FAR 52.212-1, Instructions to Offerors-Commercial items is incorporated by reference with the following changes, items (e) and (h) of the standard clause are deleted. Offerors shall include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial items Alternate III, with their offer. The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items, is incorporated by reference, which includes the following addendum 52.217-8 (in by reference), 52.217-9 (which provides that the Government may extend the term of the contract within 60 calendar days, provided that the Government shall provide a preliminary notice of intent at least 60 calendar prior to the expiration date; and services under this contract shall not exceed five (5) years), 52.232-18 (in by reference), 52.232-19 (which provides that funds are not authorized past September 30 of any fiscal year, unless written authorization is received from the Contracting Officer), 52.233-3 (in by reference), 52.244-6 (in by reference), 52.252-2, and VAAR 852.219-70 (in by reference), 852.237-70 (in by reference), 852.270-1 (in by reference) and 852.270-4 (in by reference). The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, is incorporated by reference, which shall include the following clauses: 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-3, 52.225-13, 52.232-34, 52.222-41, and 52.222-42 . Wage Determination No. 1994-2415, Revision No. 19, Revision dated May 31, 2001 applies to this acquisition. Interested offerors may submit their proposals along with a completed copy of the provision at 52.212-3 Offeror Representations and Certifications-Commercial Items Alternate III, on or before 4:30 p.m., September 21, 2001.
