This is a combined synopsis/solicitation for commercial Items prepared in accordance with the Federal Acquisition Regulations (far) subpart 12.6, as supplemented with additional Information included in this notice. This is a small business set-aside. NAICS Code is 339112 and small business size is 500 employees. This announcement constitute the only solicitation document; Proposals are being requested and written solicitation will not be Issued. The reference number for this procurement is RFQ 673-150-01 And incorporates the clauses and provisions that are in effect through Federal Acquisition Circular 97-21. This procurement is being Conducted under Simplified Acquisition Procedure (SAP): Quotes should be for the following items: Arcon (Applied Rehabilitation Concepts) or equal: 1 each Arcon BOS Basic Operating System, 1 each Arcon ST/LC Lifting Capacity System, 1 each Arcon HD Computerized Hand Dynamometer, 1 each Arcon PG Computerized Pinch Gauge, 1 each Arcon EG Computerized Goniometer, 1 each Arcon ROM Dual Inclinometer System, 1 Each Arcon Airs Automated Impairment Rating Software, 1 each Arcon DB ANA Database File, 1 each Arcon MM Multimedia System, 1 each Arcon MTM (75 unlock & Dexterity Cube, complete set-up on site and training). Equipment is to be delivered within 30 days after receipt of order. Contractor shall include warranty information. Quotes shall be FOB Destination. VAAR clause 852.210-77, Brand Name or Equal applies to this Solicitation. Offerors responding to this announcement shall provide All information contained in the FAR Clause 52.212-1, Instructions to Offeror-Commercial Items. The provision at FAR 52.212-2 Evaluation- Commercial Items also applies to this acquisition. The following factors, In descending order of importance, will be used to evaluate the quote: (1) Technical-determination of product acceptability, (2) Past performance- Delivery and (3) Price. Offers shall include a completed copy of provision 52.212-3, Offeror Representations and Certification-Commercial Items. A completed copy of this provision shall be submitted with the quote. The offeror shall also provide the Dun and Bradstreet number. The resulting contract will incorporate the requirements of FAR 52.212-4, Contract Terms and Conditions-Commercial Items and FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items. The FAR can be found on the internet at http://www.arnet.gov. Prior to acceptance of the goods or services provided under this contract It will be necessary for the government to conduct a technical acceptance Inspection. This inspection will be completed within 10 calendar days of Delivery of the goodsor services provided under this contract. A written notice Of award or acceptance offer, mailed or otherwise furnished to the successful Offeror within the time for acceptance specified in the offer shall result In a binding contract without further actions by either party. Offerors Responding to this announcement shall submit a price for this system in accordance With FAR 52.212-1 to include: a completed copy of provision 52.212-3 (Offerors Representations and Certifications), a completed copy of VAAR 852.210-71 (Guarantee) and, if bidding other than brand name, the Manufacturer, model or catalog number, and specifications of the equal.
