- This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6 and Far Part 13 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation is being issued as Request for Quote #565-03-02. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-27. The NAICS Code is 621111. The VA Medical Center, Fayetteville, N.C. has a need for services of a board certified ENT Physician and intends to solicit quotes and award a firm fixed price contract under the authority of Title 38 USC Section 8153, pending availability of funds. Contract period will be established from date of award for the period of 1 year with two one year options to extend. Estimated start date as soon as possible contingent upon credentialing of candidate. SCHEDULE OF SERVICES: 1. SCOPE: The VA Medical Center, Fayetteville, N.C. has need for services of board certified physician proficient in the diagnosis and treatment of conditions of the Ear, Nose and Throat (ENT). Physicians will provide two (2), eight (8) hours ENT Clinics per month on site at the VA Medical Center, Fayetteville, N.C. 2.PRICING SCHEDULE Coverage Rate: All-inclusive daily rate covering all fees, professional liability insurance, any housing and transportation costs involved. Provide hourly rate for ENT Physician Services $_______________. Estimated 24 clinics per year, 8 hours each. Option Year 1 provide hourly rate for ENT Physician Services $___________. Estimated 24 clinics per year. 8 hours each. Option Year 2 provide hourly rate for ENT Physician Services $_______. Estimated 24 clinics per year, 8 hours each. 3. WORK STATEMENT Contractor will provide a board-certified physician in the diagnosis and treatment of conditions of the Ear, Nose, and Throat (ENT). Services will be provided on site at the VA Medical Center, Fayetteville, North Carolina two days per month.. The average number of hours per clinic is established as eight (8) hours. The day of the week services shall be performed will be mutually agreed upon between the contractor and the Chief of Staff's office at the VA Medical Center, Fayetteville, N.C.The physician will be required to provide on site and telephone consultations for patients requiring ENT services as referred by their Primary Care physicians. Procedural services may include biopsies with local anesthetic, needle biopsies of lymph nodes and cysts, postoperative removal of splints and sutures, nasopharyngoscopies, as well as others deemed necessary by the Chief of Staff or designee. As part of care provided, contract physician is requested to prescribe for drugs on the formulary list of the Fayetteville VA facility whenever clinically appropriate. Estimated number of patients scheduled per clinic day range from 17 to 30 patients. Consultation evaluations will be patient referrals from physicians practicing at the VAMC, Fayetteville, N.C. with the intention of discharge and return to the care of the primary care physician at the earliest appropriate time. All scheduling of appointments will be processed by the PSA's located in the Outpatient Clinic of the Fayetteville, VAMC. The schedule of work for each visit will be coordinated by the Chief of Staff's office or designee. Medical emergencies during nonclinic hours will not be the responsibility of contract physicians. ON-calls service as a backup to the primary care provider for any patients on the medical wards or clinics is not required under the terms of the contract. Surgical procedures recommended by the contract physician will be referred to VA staff physicians as in current practice pattern. Any administration requirement for referral and/or transfer to another medical facility will be completed by VA personnel and approved by Chief of Staff or designee prior to transfer. The contract physician will be required to enter all reports for incorporation into the computer patient record system (CPRS) on the day of the visit and will satisfy VHA and medical center policies and JCAHO requirement for documentation. The physician must possess computer skills to access the computer patient record system (CPRS) utilized by the VA. Orientation to CPRS will be provided by the VA. Services to be performed by the contractor will be performed within the VA policies and procedures and the regulations of the medical staff by-laws of the VA facility. A contractor's employee must be present at the VA facility and must be actually performing the required services for the period specified in the contract, or the contract cost will be decreased accordingly during the billing cycle. Personnel assigned by the contractor to perform the services covered by this contract shall be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia. Personnel performing these services shall be a board certified/eligible ACLS certified and must meet the credentialing privileging requirements of the VA Medical Center, Fayetteville, N.C. Services to be performed by the contractor will be under the direction of the Chief of Staff or his/her designee at the VA Medical Center, Fayetteville, N.C. CONTRACTOR PERSONNEL SECURITY REQUIREMENTS All contractor employees who require access to the Department of Veterans Affairs' computer systems shall be the subject of a background investigation and must receive a favorable adjudication from the VA Office of Security and Law Enforcement prior to contract performance. This requirement is applicable to all subcontractor personnel requiring the same access. If the investigation is not completed prior to the start date of the contract, the contractor will be responsible for the actions of those individuals they provide to perform work for the VA. Position Sensitivity The position sensitivity has been designated as LOW RISK Background Investigation The level of background investigation commensurate with the required level of access is National Agency Check with written inquiries. Contractor Responsibilities a. The contractor shall bear the expense of obtaining background investigations. If the investigation is conducted by the Office of Personnel Management (OPM), the contractor shall reimburse VA within thirty (30) days. The contractor shall prescreen all personnel requiring access to the computer systems to ensure they maintain a U.S. citizenship and are able to read, write, speak, and understand the English language. The contractor shall submit or have their employees submit the following required forms to the VA Office of Security and Law Enforcement within 30 days of receipt: Standard Form 85P, Questionnaire for Public Trust Positions Standard Form 85P-S, Supplemental Questionnaire for Selected Positions (iii) FD 258, U.S. Department of Justice Fingerprint Applicant Chart VA Form 0710, Authority for Release of Information Form Optional Form 306, Declaration for Federal Employment Optional Form 612, Optional Application for Federal Employment The contractor, when notified of an unfavorable determination by the Government, shall withdraw the employee from consideration from working under the contract. Failure to comply with the contractor personnel security requirements may result in termination of the contract for default. Government Responsibilities a. The VA Office of Security and Law Enforcement will provide the necessary forms to the contractor or to the contractor's employees after receiving a list of names and addresses. b. Upon receipt, the VA Office of Security and Law Enforcement will review the completed forms for accuracy and forward the forms to OPM to conduct the background investigation. c. The VA facility will pay for investigations conducted by the Office of Personnel Management (OPM) in advance. In these instances, the contractor will reimburse the VA facility within 30 days. d. The VA Office of Security and Law Enforcement will notify the contracting officer and contractor after adjudicating the results of the background investigations received from OPM. e. The contracting officer will ensure that the contractor provides evidence that investigations have been completed or are in the process of being requested. The following information is provided as general information. The contractor under the terms of the contract shall furnish the work hours as mutually agreed upon in the work statement. Work hours: Monday through Friday -- 8:00 a.m. -- 4:30 p.m. National Holiday: The ten holidays observed by the Federal Government, i.e. New Year's Day, Martin Luther King's Birthday, Washington's Birthday, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving, and Christmas Day and any other day specifically declared by the President of the United States to be a national holiday. Nonworking hours (normal on-call hours) Monday through Friday, 4:30 p.m. to 8:00 a.m., and all Saturdays and Sundays. Emergency On-Call Hours: 12:00 Midnight Sunday through 12:00 Midnight the following Sunday. Work performed without prior approval of the Contracting Officer is not reimbursable. The contractor shall be responsible for protecting the personnel furnishing services under this contract. To carry out this responsibility, the contractor shall provide the following for their personnel: worker's compensation, professional liability insurance, health examinations, and income tax withholding and social security payments. The parties agree that such personnel shall not be considered a VA employee for any purpose and shall be considered employees of the contractor. Payment for any leave, including sick leave, holiday or vacation time is the responsibility of the contractor. Process of determining contract award: award preference will be as follows: 1) award to the lowest-priced responsive, responsible bidder who offers a Board certified candidate; 2) if no qualified contractor offers a Board Certified candidate, then award to the lowest-priced responsive, responsible bidder who offers a Board Eligible candidate. As part of the pre-award Contractor Responsibility determination and during the performance of the contract for the purpose of on-going Contractor competency evaluation, the VA will perform or require for evaluation from the Offeror/Contractor one or more of the following: a. Submission of information to allow evaluation of their qualifications and past performance. This information should be specific, concise, and complete and provide sufficient information to demonstrate the offeror's capacity to meet or exceed the requirements of the contract. c. Submission of evidence of experience and achievements in work similar to that required by the contract. D. Board certification or licensure that meets or exceeds standards relative to the industry being contracted. Offeror will provide the following information along with offer for each candidate they are recommending: personnel qualifications to include evidence of current licensure and credentials/privileges and current CV of candidate, most recent facility where privileges are/were held, copy of ACLS and BCLS, peer reference with current phone numbers, indemnification and Medical Liability Insurance certificate. The information required in provisions 52.212-1, 52.212-2, a completed copy of provisions 52.212-3, Offeror Representations and Certifications, Commercial Items, 852.219-70, Veteran Owned Small Business certification must be submitted with offer.
