Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation: proposals are being requested and a written solicitation will not be issued. Reference number F9ABSR12120400 is issued as a Request for Quotations (RFQ). This solicitation document and incorporated provisions are those in effect through Federal Acquisition Circular 97-15. This is a 100% small business set-aside. The size standard is 500 employees: Monthly Book Rotation Service for the period of 01 Oct 2001 through 30 Sep 2002.=20 Monthly book rotation service. To provide B-10 and B-3 book service for the Main library 1930 George Avenue, Lackland AFB, Tx. Books shall be selected by the library director or designated staff. Monthly Book Rotation Service: To provide B-5 book service for the patient Library. Books shall be selected by designated staff at WHMC/Patient Library 2200 Berquist Drive, Lackland AFB, Tx.Monthly Book Rotation Service: To provide b-5 book service for the kelly Library Branch, 37 sptg/svmgk,2261 hughes ave ste 115, San Antonio, Texas 78236-9813. Books shall be selected by the library manager or designated staff.Offerors shall include with their offers, a completed copy of 52.212-3 Offeror Representations and Certifications-Commercial Items. FAR clauses 52.212-4 Contract Terms and Conditions Commercial Items,52.212-5 Contract Terms and Conditions Required to Implement Statues or Executive orders-Commercial Items, 52.232.33 Payment by Electronic Funds Transfer-Central contractor Registration, and DFARS,252.204.7004 Required Central Contractor Registration.
