Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 and FAR Part 13 as supplemented with additional information in FAR Part 15. The announcement constitutes the only solicitation; proposals are being requested. A written solicitation will not be issued. This solicitation is issued as a Request for Proposal and is assigned number RFP 659-08-02. The solicitation document and incorporated provisions are those in effect through FAC 97-27. The procurement is 100% set-aside for small business. NAICS code is 621511; small business size standard is $10 milion dollars. The Hefner VAMC, Salisbury, NC will relocate the Charlotte CBOC to 8401 Medical Plaza Drive, Charlotte, NC on/about 10/29/01. This CBOC will provide primary care services to select veteran beneficiaries assigned to the Charlotte CBOC. VA will staff the clinic and will provide the majority of routine laboratory services. This procurement will provide one healthcare provider for STAT lab services. The Charlotte CBOC days/hours of operation are Monday through Friday 8 am to 6pm local time. The contractor must provide services, at a minimum, Monday through Friday, 8am to 4:30pm. In addition, due to our aging veterans population and the Government's requirement to obtain lab results promptly, the contractor must be located within a five (5) mile radius of the 8401 Medical Plaza Dr. address. Proof the Contractor meets this requirements must be submitted with the proposal. It is the VA's intention to solicit and award a firm fixed-price, indefinite quantity contract under the authority of Title 38 USC Section 8153, pending availability of VA funding. Contract period will be one year from the date of contract award with four (4) one-year renewal options. Estimated start date of the contract is on/about 10/29/01. There are twenty (20) Contract Line item Numbers (CLIN) for each year. The CLIN for each year will reflect the following unit cost: (1) CBC est 58/yr $______; (2) Glucose est 42/yr $_____; (3) PT & INR est 24/yr $______; (4) Urinalysis est 36/yr $_____; (5) Lytes est 14/yr $_____;(6) MG est 10/yr$_____; (7) Dilantin est 8/yr =$______:(8) Digoxin est 12/yr $_____; (9) Calcium est 15/yr $_____; (10)Phosphate est 7/yr $_____; (11) Theophylline est 12/yr $_____; (12) Blood gases est 12/yr $_____; (13)Sed Rate est 12/yr $_____; (14) Hematocrit est 12/yr $_____; (15) Cabamazepine est 12/yr $_____; (16) BHCG (pregancy test) est 6/yr $_____; (17) BUN est 12/yr $_____; (18) Creatinine est 12/yr $_____; (19) AST yest 12/yr $_____; (20) Potassium (K) est 12/yr $_____. The quantites provides are estimates based on past history and represent one year. Firms are reminded when preparing and submitting their proposal this estimate is for one year. However, the proposal submitted must reflect pricing for base year, plus four (4) option years. The VA does not guarantee any specific number of tests, nor that all listed tests will be ordered. Firms submitting proposals are to provide pricing for each test. The price should be based on the Federal Reimbursement Schedule. Frims submitting proposals must be CAP certified. The Charlotte CBOC practitions will order STAT labs on requisition forms provided by the Contract. A list of Charlotte CBOC practitioners will be provided after award. Any questions concerning the test will be referred to the practitioners at the Charlotte CBOC. Results for STAT lab will be faxed to the Charlotte CBOC within 60 minutes of the patient's arrival. Two hard copies of every lab result will be mailed within one working day of the results being faxed. One copy will be mailed to the Charlotte CBOC address (noted above). The second copy will be sent to the Chief, Pathology and Laboratory Medicine (113), Hefner VAMC, 1601 Brenner Ave., Salisbury, NC 28144. Each invoice must reflect the veterans' full name, social security number, date of service, type of test and the (contractually agreed upon) charges. A verification system wiull be implemented by the COntractor to accurately count the number of tests. The contractor may, in no way, bill the veterans and/or the veteran's insurace company for any type of test. All costs associated with the laboratory program will be borne by the VAMC Salisbury. The Austin Data Processing Center will make payment. The contractor must complete, prior to beginning service, an SF 3881, which will be furnished by the Contracting Officer. This form authorizes the VA to make payment directly to the contractor's financial institution. The Charlotte CBOC will be closed the following Government holidays: New Year's Day, Martin Luther King's Birthday, President's Day, Memorial Day, July 4th, Labor Day, Columbus Day, Veteran's Day, Thanksgiving Day, and Christmas Day. The contracting officer is the only person authozied to approve changes or modify any of the requirements under this contract. The Contractor shall communicate with the Contracting Officer on all matters pertaining to contract administration. Only the Contracting Officer is authorized to make commitments or issue changes, which will affect price, quantity, or quality of performance of this contract. In the event the contractor effects any such change at the direction of any person other than the Contracting Officer, the change shall be considered to have been made without authority and no adjustment will be made in the contract price to cover any increase in the costs incurred as a result therof. A Contracting Officer's Technical Representative (COTR) will be named in writing after award. This person will represent the Contracting Officer on matters of a technical nature. A copy of this delegation will be provided the Contractor. The information required in provisions 52.212-1, 52,212-2, a completed copy of the provisions 52.212-3, Offeror Representations and Certifications-Commercial Items; 852.219-70 Veteran Owned Small Business Certification must be submitted with the offer.
