Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number FSIS-3-B-02 is issued as a request for quotation and incorporates provisions and clauses that are in effect through Federal Acquisition Circular 97-27. This is a 100% small business set-aside. The NAICS code for this requirement is 334612, with a small business standard of 750 employees. Offerors shall provide prices for the following five items. 1. Duplication of CDs. FSIS will provide a single CD from which the contractor shall make duplicate CDs. Prices are requested for duplication in production volumes of a) 50; b) 100; c) 500; d) 1,000; e) 5,000 or more. Any charges for CD mastering shall be identified separately. If mastering prices vary by production quantity, it should be so specified. Prices for re-ordering previously duplicated CDs shall be provided for each of the identified production volumes (if the price is different from new original products, or if the price is the same, so state). 2. Color printing of CDs. Contractor must accept printing in two formats: a) positive film format, and b) QuarkExpress and PageMaker and 24-bit TIFF electronic file formats. Pricing is required for 2-, 3-, 4-, and 5-color printing. Any charges to create film from electronic files must be specified. 3. Contractor must provide color printing of CD folder and tray card inserts. Pricing is to be provided for printing of CD folder and tray cards for the following volumes: a) 500; b) 1,000; and c) 5,000 or more. Pricing shall be provided for the following printing choices: a) 4/0; b) 4/1; c) 4/2; d) 4/3; and e) 4/4 (note that the first number is the outside color; the second number is the inside color). Pricing is required for creation of film from electronic files for the following color combinations: a) 4/0; b) 4/1; c) 4/2; d) 4/3; and e) 4/4. If applicable, pricing shall be differentiated by a) tray cards; b) 2-page CD cards, and c) 4-page CD cards. 4. Jewel cases. Contractor must provide hard plastic jewel cases with placement of the CD in jewel case, insertion of CD folder and tray card inserts, and shrink wrap of jewel cases. Offeror shall provide pricing for this item. 5) Delivery. Contractor must ship the product within two calendar weeks of receipt of the original CD and printing materials from FSIS. The contractor must ship product to multiple locations in quantities of 50 or more to individual locations. Contractor should indicate the amount of the handling charge per shipment, if any. The provision of FAR 52.212-1, Instruction to Offerors-Commercial Items, applies to this acquisition with no addenda. The Government will issue a Blanket Purchase Agreement (BPA) resulting from this solicitation to the responsible offeror whose offer conforming to this synopsis/solicitation will be most advantageous to the Government in terms of price, quality, and past performance. Prior to award, the Government reserves the right to review an offeror's sample product prior to issuance of a BPA. The Government reserves the right to issue multiple BPA's. Offerors shall identify how the offered item meets each of the government's requirements listed herein. Responses may include literature, brochures, and other such information corresponding to the required items which demonstrates the offeror's capabilities. Offerors shall provide at least three references of their most recent relevant projects. Offerors are to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. The clauses at 52.212-4, Contract Terms and Conditions -- Commercial Items applies to this acquisition with no addenda. The following FAR clauses in paragraph (b) of FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items will apply to the resultant contract: 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-3, and 52.225-13.
