Description

The GLAC (Great Lakes Acquisition Center) is soliciting quotations to provide VA Iron Mountain, Michigan with one (1) part- time Primary Care Physician, M.D. services for its beneficiaries in the upper peninsula of Michigan. Place of contract performance is the Menominee, Michigan Community Based Outpatient Clinic (CBOC). Anticipated Period of Performance is January 31,2002 through January 30,2003 with one (1) option year renewal period, at the Government's discretion. Contract type is firm-fixed price. Cost per Hour to be all inclusive (all labor, supervision, transportation, per diem, insurance etc.). Work hours include 16 regular work hours per week (8:00AM-4:30PM), two days per week, 52 weeks per year. Anticipated award date is December 15,2001. This solicitation is 100% set-aside for Very Small Business. Electronic version and hard copy of solicitation will be available on or about September 28, 2001. This solicitation is issued under the authority of Public Law 104-262 and 38 USC 8153. This procurement is being conducted under the Subpart 13.5 test program where simplified acquisition procedures apply and the conduct of the procurement will be in accordance with Parts 12, 13, or 15 of the FAR or some combination thereof.
