Description

Full preventive maintenance service contract for SONOS 5500 Imaging System Model 2424-25A, located at the Department of Veterans Affairs Medical Center, in building 7, Room 123 at the Alexandria, Louisiana facility. This contract will include on-site corrective repairs during normal working hours of 8:00AM to 4:30PM, Monday through Friday, excluding federal holidays. The contractor shall respond via telephone within two (2) hours after receipt of trouble calls, and provide on-site service within 24 hours, when needed. Equipment shall be operational within 48 hours. Maintenance service to include all parts, labor, repair kits, upgrades and rebuilds as issued by the manufacturer. Expendable items such as recording paper, videotapes, electrodes, film, and optical discs are not considered part of this agreement. Parts will not be replaced without prior approval of the COTR. All components replaced will be warranted against defects and/or failure during their normal commercial warranted period. Any call back service for the same malfunction shall be at no charge to the Government during this warranty period. Service is also to include quarterly inspections, which will be performed during the months of November, February, May, and August. Procurement is restricted to Small Business NAICS 811219. Requests for hardcopy solicitation must be in writing to the Contracting Officer and must be accompanied by a non-refundable check for $10.00 made out to the US Dept of Veterans Affairs. No telephone request will be accepted.
