Description

The Contractor shall perform Routine and Stat Laboratory Test on patients or specimens referred by the DVA Community Based Outpatient Clinic(CBOC), 238B Hospital Drive, Ukiah, CA 95482 for the period of January 1, 2002 through December 31, 2002. The Contractor will provide personnel, facilities, supplies, and equipment necessary to perform the required services which include but are not limited to: 1. Obtaining specimen(s) for laboratory test(s), 2. Perform laboratory tests on specimen(s), 3. Fax results of the Stat laboratory test to the Ukiah CBOC. No telephone or email request will be accepted. No web-based offers will be accepted. The offeror is responsible for downloading all documents and amendments without further notice from the DVA. All questions must be in writing and may be faxed to (415) 750-2170.
