- Maintenance on Hospital Elevators- Elmendorf Air Force This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR Subpart 12.6. PLEASE READ THIS ENTIRE NOTICE CAREFULLY AS IT CONSTITUTES THE ONLY NOTICE THAT WILL BE ISSUED. . This is a Sole Source acquisition to Thyssen Krupp Elevator, Anchorage, AK, 99518. This is for the purchase to maintain and service the Emed Teleradiology Systems. This announcement constitutes the only solicitation: a quote is being requested and a written solicitation will not be issued. Solicitation number F65501-02-Q0006 is issued as a Request for Quotation (RFQ). The incorporated document provisions and clauses are those in effect through Federal Acquisition Circular 97-27. The Standard Industrial Classification (SIC) Code is 1796. The NAICS Code is 235950. PROJECT: SCOPE OF WORK. DIVISION I -- GENERAL REQUIREMENTS 1. DESCRIPTION: Contractor shall provide all labor, materials, equipment, transportation, tools for inspecting, reporting, servicing, and all preventive maintenance inspections and repairs necessary to keep the 12 hospital elevators functioning in a safe and reliable condition. Contractor shall comply with the requirement stated in the technical provisions. Reference attached Examination and Lubrication Agreement. 2. EQUIPMENT: Five Dover Hydraulic Passenger Elevators and seven Dover Geared Traction Elevators. 3. EQUIPMENT LOCATION: 3rd Medical Center 5595 Zeamer Avenue Elmendorf AFB, AK 99506 4. HARDWARE AND SOFTWARE PROGRAM: N/A. 5. PERFORMANCE PERIOD: Annually starting 01 October 2001 6. FREQUENCY OF SERVICE: once per month for each elevator. 7.RESPONSE TIME: N/A. 8.REPAIR SERVICE HOURS: During normal duty hours from 7:30 AM through 4:30 PM, Monday through Friday, except holidays observed at the Government. 9. REQUIRED SERVICES: Service includes the cleaning and lubrication of the machine, motor, interlocks, and controller; the lubrication of guide rails and the making of minor adjustments at the time of regular examination for each elevator. 10. REPLACEMENT PARTS: Lubricants and cleaning materials are the responsibility of the contractor. Any required repair or replacement parts are the responsibility of the owner. 11. QUALIFICATIONS: Contractor shall furnish supporting documentation as evidence of their capability to completing the requirements of this contract. 12. PERMITS: N/A 13. OPERATIONAL AND TECHNICAL DOCUMENTATION: N/A 14. WORK LOCATION: Building 5955 Zeamer Ave., Elmendorf AFB, AK. 15. TRAVEL EXPENSES: Included as part of the monthly fee. 16. REPORTS: N/A 17. QUALITY CONTROL: The Contractor shall maintain Quality Control standards commensurate with elevator industry standards. 18. FIRE AND SAFETY REQUIREMENTS: The contractor and contractor representatives shall comply with Air Force, Federal, and State fire and safety regulations at all times while on government property. A copy of these regulations can be obtained from each department. Contractor shall comply with the no smoking policy within the 3rd Medical Center. 19. TESTING AND INSPECTIONS: Perform all inspections, test parts and systems specified herein, as stated in applicable codes and standards. Contractor shall provide all necessary equipment, material and labor necessary to properly perform the above. 20. UTILITIES: Reasonable amounts of water, electricity, and other available utilities will be furnished by the government at no cost to the Contractor. The Contractor shall safeguard the Government's utilities from damage. All temporary connections shall be the Contractor's responsibility. The Contractor shall promptly remove all temporary connections upon completion of work. Delivery terms shall be FOB Destination, Elmendorf AFB, Alaska 99506. The following provisions and clauses are incorporated and shall remain in full force in any resultant purchase order; FAR Part 52.212-1 Instructions to Offerors-Commercial Items, is incorporated by reference. The provision at 52.212-2 Evaluation of Commercial Items, applies to this solicitation. The Government reserves the right to make award on the initial quote received without discussions. Each offeror shall include a completed copy of the provision at FAR 52.212-3 Offeror Representations and Certifications-Commercial Items with its quote. FAR Part 52.212-4 Contract Terms and Conditions-Commercial Items, is incorporated by reference and applies to this acquisition. Note the following addenda to FAR 52.212-4: Incorporate DFARS 252.204-7004 Required Central Contractor Registration. FAR 52.222-48 Exemption from Application of Service Contract Act Provisions for Contracts for Maintenance, Callibration, and /or Repair of Certain Information Technology, Scientific and Medical and/or Office and Business Equipment -- Contractor Certification. FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Order?Commercial Items (XXX 2001) (DEVIATION). DFARS 252.222-7000 Restrictions on Employment of Personnel (Mar 2000)(IAW DFARS 222.7003). This is a DO rated order with priority identification symbol of S10. A Firm-Fixed Price purchase order will be issued under the Simplified Acquisitions Procedures. Submit signed and dated offer to 3CONS/LGCB at or before 02 November 2001 3:30 PM Alaska Standard Time. Quotation shall be signed, dated and submitted on your company?s letterhead or quotation forms. Quotations may be submitted via fax.
