Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Sol. No. 438-12-02 is issued as a request for quotation (RFQ). Solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-01. NAICS code 611430, size standard $5.0 million applies. Services required: Career Development and Leadership Training for approx. 400 VISN 13 first-line supervisors, managers and team leaders. Items to be delivered: Promotional and registration materials; Paper and electronic brochure describing program purpose, program goals, structure of program and proposed timetable for delivery; confirmation letter format; pre-work assignment including preparatory information in support of the first classroom experience; Participant workbook and appropriate reference materials to support classroom experience, skill practice case studies and post-classroom homework and Powerpoint presentation and supplies for three learning sessions (all of above with exception of learning materials for sessions 2 and 3 are due 12/15/01). Skill, knowledge and/or behavior outcomes measurement assessment tracked and reported throughout implementation of career development series. Certificate of completion of career development series. Learning instruments to assess leadership preferences. Laminated job aids; Master copy of all training and support materials in hard copy and on disc. Learning sessions will be conducted by contractor personnel at all VISN 13 facilities during 2002 -- Minneapolis, Sturgis, Hot Springs, Sioux Falls, St. Cloud and Fargo. On completion of scheduled training, all training materials will become VA property. This will be a firm fixed-price contract. Factors for evaluation of proposals include: past performance history (include three corporate or Government references with phone numbers); capability to meet the Government's requirement (contractor to identify current workload and experience in designing and delivering leadership development curriculum that reinforces the VHA High Performance Development Model); and price. Award will be based on best value to the Government. Evaluation may be made with or without discussions. A response to the requirements shall be submitted on contractor letterhead or formal quotation form and must address all requirements listed in a separate technical proposal. Proposal must contain the name, title and signature of person authorized to submit proposals on behalf of the contractor. The following provisions and clauses shall apply to this solicitation: 52.252-2 Clauses incorporated by Reference. This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text.
