Description
VA Gulfcoast Veterans Healthcare Systems, 400 Veterans Avenue, Biloxi, MS intends to negotiate a sole source, fixed price contract with Marconi Medical Systems to perform prevention maintenance inspections and repairs on the nuclear camera systems, which includes the AXIS 2-head and the ARIX 3-head. This Medical Center intends to solicit and negotiate with only one source under the authority of FAR 6.302-1.=20 See Note Number 22. This is a combined synopsis/solicitation for a commercial service, prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This notice constitutes the only solicitation; a written solicitation will not be issued. Solicitation number 520-18-02 is assigned and shall be referenced on any quotation. The NAICS is 811219.=20 Contractor shall provide the following services in accordance with all terms, conditions and provision of the solicitation for the period November 5, 2001 through September 30, 2002 with two (2) renewal option years from the date of the award. Requirements: 1. Contractor to provide all labor, parts, and materials necessary to provide quarterly preventive maintenance inspections on the referenced nuclear camera systems above. Each inspection will be performed in accordance with established manufacturer's procedures and shall include an inspection of all the equipment and its components. Contractor to schedule and perform inspections and all maintenance between the hours of 8AM and 4:30PM, Monday through Friday, exclusive of Federal holidays, unless otherwise specifically requested. 2. Contractor to schedule the quarterly preventive maintenance inspection with the Biomedical Program Manager and/or his representative @ 228/523-5007, 2-weeks in advance of actual inspection. All repairs and quarterly preventive maintenance shall be performed by a manufacturer trained technician. Provide certification of training with bid. 3. All interim maintenance, labor, and repair parts will also be included in this contract and will cover emergency callbacks necessitated by failure of the equipment to perform at required manufacturer's standards. B. Special Requirements: 1.=20 This contract will cover the cost of all replacement parts, including system detector and crystal. Replacement parts must be original equipment manufactured or remanufactured parts and/or components by the original manufacturer. No substitute parts will be acceptable. Consumable parts will be the responsibility of the Government. 2. All labor, repair, and parts costs will be included in this contract for the equipment inspections, adjustments, lubrication, installation of replacement parts, and testing, while providing the scheduled preventive maintenance inspection. 3. Contractor shall report to Biomedical Engineering Section upon arrival prior to accomplishing the preventive maintenance inspection/repairs/callbacks, etc. They will indicate time of arrival, date, company name, description of the problem, and equipment to be serviced. After repairs, the contractor will complete a service report indicating the labor and/or parts required to affect the repair, time of departure, and obtain a signature from the Biomedical Program Manager and/or his representative.=20 4. This contract does not include labor or materials for service lines and fittings, equipment not set forth on this schedule, or operating problems related to environmental conditions, e.g. electrical, steam, water, and/or ventilation that does not comply with equipment specifications. This also applies to improper operation of the equipment by operating personnel. 5. Repairs made by the contractor, which are the result of neglect or improper operation of the equipment by the medical center, will result in issuance of a separate purchase order by the Government. Contractor must obtain a purchase order in this respect prior to effecting such repairs.=20 Contractor must notify the Biomedical Program Manager and/or his representative prior to this type of repair and secure a purchase order before proceeding further. 6. Productivity Partnership Agreement (PPA): In addition to the coverage defined above, this agreement includes the following five (5) items without additional charge for materials or labor. These items will be provided for the entire term of this agreement provided the terms of the agreement have been met. a)=20 Following implementation into Marconi production, new releases and updates to the Operating (Baseline) software will be provided and installed on the main integrated system computer and all related workstation computers. Updates are provided on an annual basis and new releases are provided as they occur. b) Following implementation into Marconi production, all hardware necessary to maintain the main integrated system computer (CPU) at current production performance levels will be installed. c) A 5% discount, incremental to prevailing field level discounts, is available for any software or hardware upgrade option purchased from Marconi Medical Systems current published offerings for the system(s) covered by this PPA. d) As available, installation of Site Manager Server =99 hardware and enable service technology providing advanced remote diagnosis and repair. This hardware remains Marconi Medical Systems property and is subject to the "Service Materials" terms defined by this agreement. e) Enrollment in the Marconi Nuclear User's Group Team Room is included. 7.
