Description

This is the second modification to a prior notice posted on 7 Nov. 2001. The DD Form 2345 on the US Army TACOM-ARDEC Procurement Website has been revised. To find the form, go to www.dlis.dla.mil/jcp. At the top of that page open Documents on the blue bar. There is a revised DD Form 2345. If you don't have a DOD Certification/Registration Number send the DD Form 2345 to the US/Canada joint Certification Office address on the form. A COPY OF THE COMPANY'S STATE/PROVINCIAL LICENSE, INCORPORATION CERTIFICATE, SALES TAX IDENTIFICATION FORM OR OTHER DOCUMENTATION WHICH VERIFIES THE LEGITIMACY OF THE COMPANY MUST ACCOMPANY ALL DD FORMS 2345. These documents are listed in the NOTE below the form. There is also a pamphlet on that website providing guidance on the US/Canada Joint Certification Program. Respondents are advised that an employee identified below of the Mitre Corp., 7515 Colshire Dr., McClain, VA 22102 a Federally Funded Research and Development Center, may serve as a technical advisor in the source selection process. This individual will be authorized access to only those portions of the White Paper data and discussions that are necessary to enable them to perform their respective duties. This firm is expressly prohibited from competing on the subject acquisition and from scoring or ranking of White Papers or recommending the selection of a source. James Horn, Telephone 860-342-3317 In accomplishing their duties related to the source selection process, the aforementioned firm may require access to proprietary information contained in the respondent's White Paper. Therefore, pursuant to FAR 9.505-4, this firm must execute an agreement with each respondent that states that they will (1) protect the respondent's information from unauthorized use or disclosure for as long as it remains proprietary and (2) refrain from using the information for any purpose other than that for which it was furnished. To expedite the evaluation process, each respondent must contact the above company, ATTN: Ms. Joan Konz to effect execution of such an agreement prior to submission of proposals. Each respondent shall submit copies of the agreement with their White Paper.
