Description

The Fleet and Industrial Supply Center (FISC) San Diego, has a requirement for medical physical examinations for the Navy Reserve Recruits within the Southwest Region. IMPORTANT NOTICE: DFARS 252.204-7004 "Required Central Contractor Registration" applies to all solicitations issued on/after 01 June 1998. Lack of registration in the CCR database will make an offeror/quoter INELIGIBLE FOR AWARD. Please ensure compliance with this regulation when submitting your quote. This is a combined synopsis/solicitation for commercial services prepared in accordance with FAR 15 and the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. The Reque st for Proposal (RFP) number is N00244-02-R-0014. This is an unrestricted RFP. The requirement is for a Firm Fixed Price Indefinite Delivery Indefinite Quantitiy (IDIQ) type contract for a Base Year and one Option Year. The contract will contain a guaranteed minimum of 10% of the Base Year quantity and a contract maximum of 100% of the Base Year quantity. The following separately priced Contract Line Items (CLINs) will apply to the contract: Base Year: 0001 Category I: Enlisted Physical; Contractor shall conduct a basic physical in accordance with the requirements set forth in the Statement of Work (SOW) as incorporated herein. Quantity 1,000 (per year) 0002 Category II: Labs Only (as required). Quantity 100 (per year) 0003 Category III: Officer Physical; Complete physical and labs are required. Quantity 124 (per year) Option I: 0004 Ca tegory I: Enlisted Physical; Contractor shall conduct a basic physical in accordance with the requirements set forth in the Statement of Work (SOW) as incorporated herein. Quantity 1,000 (per year) 0005 Category II: Labs Only (as required). Quantity 100 (per year) 0006 Category III: Officer Physical; Complete physical and labs are required. Quantity 124 (per year) Delivery shall be FOB Destination. Deliver to: NAVRESCRUIT AREA PACIFIC (N41), 960 North Harbor Drive, San Diego, CA 92123-0007. The government intends to make a single award to the responsible offeror whose offer is the most advantageous to the government considering price and price-related factor s. Provision 52.212-2, Evaluation-Commercial Items, applies with paragraph (a) completed as follows: Award will be made to the offeror that meets the solicitation's minimum criteria for technical acceptability at the lowest price. To be determined technically acceptable, the offeror must submit past performance information that supports proof of the offeror's capability to perform the required services. Past performance data may consist of prior government contracts and orders within the past three years where similar services have been provided. Provide personal points of contact and phone numbers to assist in the validation of the claimed performance. Navy Reserve Recruiters have a requirement for medical physical examinations for new reserve recruits, enlisted and officer personnel. An Indefinite Delivery, Indefinite Quantity Contract (IDIQ) with government purchase card methods of payment is anticipated. The Categories are as follows: Category 1 Enlisted Tests * Blood Pressure * Hearing * Vision * Height/Weight (Navy Standards) * Blood Typing Labs (as required) * HIV * Urinalysis -- (Albumin and sugar (Dip Stick). * PAP -- if requested by examining physician. * Pregnancy -- Non-Prior Service Enlistments/Female Officers only. Additional Tests for applicants over 40 (Mandatory) * EKG * Tonometr y (Glaucoma Test) * Lipid Profile (Cholesterol, Triglycerides and High Density Lipoproteins [HDL]) * Mammograms for females. * Stool Guaiac Additionally, items below maybe required for some specific job specialties. * Vision -- Correctable to 20/20 -- Color Vision. -- Depth Perception Category 2 Labs Only (as required, see above) Determination will be made by the Physician Category 3 Officer Physical (Complete Physical and Labs are required)
