Description
This is a combined synopsis/solicitation for a commercial service prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; a written solicitation will not be issued. The solicitation number is USZA22-02-R-0051, and is issued as a Request for Proposal (RFP). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-27. The North American Industry Classification System (NAICS) code applicable to this acquisition is 611519. PROPOSAL INSTRUCTIONS: Offerors must submit proposals, which can be their standard school/course materials, which demonstrate that their proposed course of instruction is fully compliant with all requirements in the Statement of Work. In addition, Offerors must submit resume(s) of the proposed course instructor(s). Offerors must be a state-licensed Emergency Medical Training Paramedic Training Center to be eligible for award. Offerors must submit proof that they are licensed as part of their proposal. Offerors must submit pricing for CLINs 0001, 0002 and 0003 as a ?price per student? that includes all costs, for example tuition, student fees, lab supplies and textbooks. Proposals are due no later than December 14, 2001 at 2:00 PM. Proposals must be submitted in electronic format via email to hollandm2@socom.mil (with a copy to spurlik@socom.mil). CONTRACT LINE ITEMS: There will be three classes per year, with 30-35 students per class. CLIN 0001 Base Year Jan 2001-31 Dec 2001 CLIN 0002 Option Year 1 Jan 2002-31 Dec 2002 CLIN 0003 Option Year 2 Jan 2003-31 Dec 2003 ** BEGIN STATEMENT OF WORK ** 1. Scope Of Work 1.1 The contractor shall provide training for qualification as Emergency Medical Technician Paramedics (EMT-P) within the new 1998 National Highway Traffic Safety Administration's (NHTSA) curriculum guidelines and the certification standards of the National Registry of Emergency-Medical Technicians (NREMT) to challenge the NREMT examination. 1.2. Each contracted year will consist of a total of three (3) classes. The total number of students per class will range from a minimum of 30 to a maximum of 35. 1.3. The classes will consist of the following types of personnel, who will be classified as "students" during training: * U.S. Army Special Forces Medical Sergeants (SFMS) of (Military occupational Specialty (MOS) 18D), * U.S. Army Healthcare Specialist of (Military occupational Specialty (MOS) 91-B or W), * U.S. Navy SEALS (Naval Enlisted Classification (NEC) 8491, 8492), * U.S. Air Force Special Operations Forces Pararescuemen (Air Force Specialty Classification (AFSC) 1T2X1, and * Independent Duty Medical Technicians (Air Force Specialty Classification (AFSC) 4N0X1 and 4F0X1. 2. Requirements 2.1. Student pre-registration shall be completed not less than one month prior to the beginning of each class. Student in-processing to the Contractor's facilities will begin no less than two days prior to each class convening date. 2.2. The Course of Instruction (COI) shall be no more than eight (8) weeks consisting of required didactic/classroom training and field experiential training. This field training shall be with local Emergency Medical Services (EMS) ambulances, and clinical experiential training with local hospital and medical institutions. The training shall provide patient exposures for practical hands-on applications under the supervision of privileged, certified and/or licensed clinicians and practitioners and field training will be conducted in advanced life support (ALS) equipped ambulances and helicopters. The training must meet the certification requirements of the NREMT. Clinical experiential training must include performing training rotations in both a pediatric and an adult level II trauma center. 2.3. The Contractor shall coordinate and administer the National Registry EMT Basic and EMT Paramedic testing upon completion of each SOF Paramedic course. 2.4. The Contractor shall provide to USSOCOM individual academic performance ratings and overall class performance analysis. 2.5. The training course must be academically reviewed and certified with the institution/contractor as required to meet the standards of training for EMT-P, taking into account training already evaluated and received during the "student's" prior MOS, NEC or AFSC training. 2.6. Hours of operation. Normal hours will be those hours required to execute the course of instruction, which may include week days, week ends, day and night hours, which are within the guidelines of the institution and conducive to the effective oversight and execution of the training. 3. Contractor Personnel 3.1. Program Manager. The contractor shall designate a program manager, within the executing department, responsible for the execution and performance of this contract. The program manager will serve as the focal point for communications between the Contractor and the Government Contracting Officer and requiring activity official. The Program Manager may be the same employee that is responsible for the execution and performance of training. The contractor shall identify in writing to the Contracting Officer, the Program Manager and an alternate(s) who shall act for the contractor when the manager is absent within one week of notification of award. 3.2. The Program Manager, or alternate(s), must be available at all times during the execution of the COI. 3.3. The Program Manager, and alternate(s), must be able to read, write, speak and understand English. 3.4. No employee, or agent, of the U.S. Government shall act as, or be considered, an employee, or agent, of the providing contractor. No employee, or agent, of the providing contractor shall act as, or be considered, an employee or agent of the U.S. Government. 3.5. The contractor shall continually ensure that all instructor personnel have current and valid professional certifications required by the institution, state and national accreditation agencies. 4. Facilities. No Government furnished facilities will be furnished. All training shall be at the contractor's facility. Contractor shall ensure, within the best conditions possible, that while within the institution, a safe and secure environment is provided for the "students". 5. Information Security. Contractor shall not release any information to a third party on any "student". Information which is personal in nature, academic progress reports, discipline reports, and test results, shall not be released to any other party except the Office of the Command Surgeon, Medical Training Division, USSOCOM. 6. Quality Control 6.1. The contractor shall establish and maintain a Quality Control Plan to ensure that the requirements of the contract are provided as specified. A copy of the Plan will be provided to the Contracting Officer within one week of notification of award. 6.2. The Plan shall include, at a minimum: * An inspection system, which will be monitored by the contractor, in coordination with the contracting institution during each execution period of the Course of Instruction. * On-site records shall be available to the contracting institution, as the Government representative and interested party, and all accreditation agencies and institutions, such as the NREMT, which require access for inspection to maintain the contractor's accreditation. 6.3. Quality Assurance. The Government will evaluate the contractor's performance through methods of surveillance which include on-site visits and evaluations and institutional academic reports. The contractor shall provide all academic reports to the Office of the Command Surgeon, Medical Education and Training Division, who will monitor pass/failure rates of the course and successful challenges to the NREMT examination. The contractor shall provide USSOCOM with all written and practical skills examination results from the NREMT Basic and Paramedic testing periods. 6.4. COntracting Officer?s Representative (COR) Evaluation Meetings. The contractor may be required to meet with the COR periodically (tri-annually, conducive to each class session) to review and determine performance of the program and its execution, and to determine if adjustments must be made to improve performance and maintain the standards of accreditation and certification for testing under the criteria of the NREMT. ** END STATEMENT OF WORK ** CONTRACT CLAUSES AND PROVISIONS: The following clauses and provisions apply to this procurement, and there are no addenda: 52.212-1 52.212-2 52.212-3 52.212-4 52.212-5 252.212-7000 252.212-7001 52.216-18 52.216-19 52.216-21
