Description
(I) This is a combined synopsis/solicitation for commercial services prepared in accordance with the format Prescribed in Subpart 12.6 of the FAR, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. NO OTHER DOCUMENTS WILL BE ISSUED. (II) This solicitation is issued as Request for Quotation (RFQ) 40804-2B1026. (III) This solicitation document and incorporated provisions are those in effect through Federal Acquisition Circular (FAC) 97-27, dated August 7, 2001. (IV) This solicitation will be issued as a 100% total set aside for small business firms. The North American Industry Classification Systems (NAICS) code is 621999. (V) (A) Service to be performed: Plans and provides a completely integrated pharmacy program, and coordinates these programs with all other departments within the institution. Responsible for the records and reports related to the purchase, receipt, and disposition of drugs and related supplies. Responsible for maintaining and monitoring controls over DEA controlled substances, and ensured compliance with all applicable laws and regulations. Purchases, stores, and dispenses all needles/syringes, and maintains records regarding these activities. Develops and monitors a pharmacy orientation program for new pharmacists, technicians, physicians, physician assistants, and nurses. Prepares and submits purchase requests for needed drugs and supplies. Due to the Prime Vendor Contract purchasing systems, incumbent will operate autonomously in the purchasing of drugs and supplies, and must be prepared to defend such purchases when required. Supervises and assists in daily dispensing as necessary to maintain programs. Responsible for the provision of services in response to special and unusual diagnoses and treatments. This includes, the manufacture and preparation of medications not commercially available. Responsible for the coordination of pharmacy services within the complex environmental and organizational structure of separate facilities that complicate the provision of Pharmacy Services at a federal prison. Responsible for supervising and coordinating the pharmacy outpatient service. Also, responsible for servicing unique dispensing situations, such as "pill line" and segregation units. Responsible for pharmacy "interventions", whereby all provider orders are reviewed for drug-drug, drug-food, and drug-disease interactions, inappropriate/inaccurate usage, therapeutic duplications, omissions, errors, and inappropriate therapy. Responsible for a pharmacy "Continuous Quality Improvement Program", that addresses problem trends, develops measurement criteria, and seeks ways to rectify problems. Any individual working within the confines of the U.S. Penitentiary must undergo a background investigation and drug test prior to being permitted inside the institution. In addition, a 3-4 hour orientation class must be completed prior to beginning the contract work. The time spent in this class is reimbursable to the contractor at a rate negotiated with the government at time of the award of the contract. The contractor will not be supervised by the Government, however, work will be monitored in order to verify satisfaction of contract requirements. The offers received will be evaluated on price and other factors considered that will be most advantageous to the Government. The Government may award the contract without discussions,, therefore, offerors are instructed to submit their best offer initially. A site visit is not scheduled. (VII) FAR provision 52.212-1, Instructions to Offerors- Commercial Items (Oct 2000), is hereby reference. The following addenda are included by reference: At paragraph C, Period of Acceptance of Offers, offerors must agree to hold the prices submitted for sixty (60) calendar days from the date specified for receipt of offers. At paragraph H, Multiple Awards, the Contracting Officer has determined multiple awards are not applicable in this solicitation, and the paragraph is hereby removed in its entirety for the purpose of this solicitation. (VIII) FAR 52.212-3 Offerors Representations and Certifications- Commercial Items will be completed prior to award. (IX) FAR provision 52.232-33, Mandatory Information for Electronic Funds Transfer Payment applies to this solicitation. (X) Offers will NOT be accepted by fax. Offers must be received in Contracting Office no later than 2:00pm, Central Time, December 17, 2001. Envelopes must be clearly marked with solicitation number. THIS IS NOT A SEALED BID.
