- Description
- The Federal Bureau of Prisons, Federal Correctional Institution (FCI), Ashland, Kentucky, intends to award a Firm-Fixed Price, Requirements type contract with a single twelve (12) month base period and four (4)option periods of twelve (12) months each. The contractor must supply the services in accordance with this Request for Quotation (RFQ). The work schedule will normally consist of three three-hour visits per month, a visit is defined as three (3)hours or an evaluation of at least twelve (12) patients. These services may be provided on an as needed basis, the contractor is subject to a call for services from FCI Ashland, Kentucky. The contractor will be responsible for the following: 1. Performing eye examinations and refractions of patients referred by medical staff, to include but not limited to, providing diagnosis, treatment and on-site follow-up for findings of systemic diseases such as glaucoma, diabetes, hypertension, staging of retinopathy etc. 2.Provide clinical description of fundoscopic findings for any systemic disease or conditions. 3.Submitting written reports of comprehensive evaluations, and/or plans of treatment for all patients evaluated. These reports may be hand written or typed and must be verified with the signature of the contractor. 4.Providing on-site urgent care as requested as clinically indicated. 5.Ordering and fitting eyeglasses based on the findings of the examination. 6. Timely verification of accuracy of prescriptions prior to dispensing glasses to the inmate. Making adjustments prior to dispensing to the inmate and providing these services at the institution. 7.Performing tonometry. Provide notification, treatment or referral for abnormal findings. 8.Making recommendations for further evaluations or testing as clinically indicated. 9. Informing the Health Services Administrator of any discrepancies or equipment needs in order to provide the required service. 10.Providing annual verification of licensure and/or registration (renewals must be provided prior to expiration). Provide medical liability insurance coverage, continuing education and training updates. Provide three (3) letters of reference to verify professional competency on initiation of contract. 11.Providing staff education and annual competency verification on the use of visual fields machine testing, lensometer and the auto refractor unit. The work schedule will normally consist of three three-hour visits per month, a visit is defined as three (3)hours or an evaluation of at least twelve (12) patients. A session is defined as one hour. Sessions not to exceed 108 sessions in one year. These services may be provided on an as needed basis, the contractor is subject to a call for services from Federal Correctional Institution, Ashland, Kentucky. This is a non-personal services contract and not a personnel appointment. The solicitation is 100% set aside for small business. Contract to exceed 12 months each.
