- Provide healthcare resources in accordance with Public Law 104-262 and 38 U.S.C. 8153. Contractor agrees to provide a Speech Language Pathologist, Certified through American Speech and Hearing Association (ASHA) as requested by the VA Roseburg Healthcare System (VA). Contractor shall provide a Speech Language Pathologist, Certified through American Speech and Hearing Association (ASHA) within two (2) hours upon notification from VA. CONTRACT START DATE: FEBRUARY 4, 2002 THROUGH FEBRUARY 3, 2003 Contractor to provide Speech Language Pathologist, Certified through American Speech and Hearing Association (ASHA) with minimum one (1) year diagnostic and clinical experience in assessment and treatment of speech and language disorders and dysphagia in acute hospital setting. Contractor shall provide Speech Language Pathologist within two (2) hours of notification from VA. If contractor cannot provide a Speech Language Pathologist, contractor shall notify VA and VA will procure service elsewhere. Contractor may be required to provide Speech Language Pathologist on weekends or holidays. Speech Language Pathologist must have had academic training and clinical experience in the assessment and treatment of adult dysphagia, mechanical and neurological. Speech Language Pathologist will see a wide range of diagnoses on inpatients and outpatients to include but not limited to CVA, degenerative neurological diseases, and s/p surgical resections/reconstructions. The Speech Language Pathologist must: a. Have an unrestricted license in a US state, territory or b. US commonwealth; b. Speak English proficiently; c. Provide evidence of demonstrated current clinical competence to perform appropriate services as described herein; d. Be a US citizen; f. Provide evidence of current professional liability insurance; g. Evidence of current privileges (if applicable); h. Names of most current employer(s) and two peer reference contacts; i. Current copy of BCLS certification; j. Be a team player; k. Have experience in radiographic studies. l. Must be computer literate, as VA Roseburg Healthcare System utilizes a computerized patient medical record; m. Must have a minimum of one (1) year diagnostic and clinical experience in assessment and treatment of speech and language disorders and dysphagia in acute hospital setting. Speech Language Pathologist will submit results of quarterly peer reviews of clinical competence, which will also include age related competencies. All contractor employees who require access to the Department of Veterans Affairs' computer systems shall be the subject of a background investigation and must receive a favorable adjudication from the VA Office of Security and Law Enforcement prior to contract performance. This requirement is applicable to all subcontractor personnel requiring the same access. If the investigation is not completed prior to the start date of the contract, the contractor will be responsible for the actions of those individuals they provide to perform work for VA.
