C-TITLE: RENOVATE WARD C, BUILDING 1 SOL 644-22-02 DUE 020102 POC Contracting Officer, Donna Falcon, Carl T. Hayden VA Medical Center, Phoenix, AZ 85012 (602) 212-2171. The Carl T. Hayden VA Medical Center, 650 E. Indian School Road, Phoenix, AZ 85012 has a requirement for professional Architect/Engineering Services to provide Contract Drawings, Specifications, Construction Cost Estimate and subsequent Construction Period Services for Project 644-210, Renovation of Ward C on the fourth floor of Building No. 1 of the VA Medical Center. The A/E shall design for the Renovation of Ward C, approximately 10,000 SF, on the fourth floor of Building No. 1 as identified by the VAMC Phoenix, Engineering Department. Demolition including asbestos abatement, removal of walls, ceilings and flooring as necessary. All areas involved will be designed for mechanical, electrical, finishes and be site ready for any new equipment to be installed. Existing space shall be utilized to the maximum extent. The project design will correct patient privacy and space layout deficiencies associated with the existing space. All design work shall be accomplished in accordance with VA guidelines and specifications, which are available on the Internet at http://www.va.facmgt/standards. A/E design work shall be in accordance with NFPA, OSHA, DER, NEC, 101 Life Safety Code, and other State/Federal/or local codes and/or regulations. All final documents will be issued to the VA within 164 calendar days after issuing Notice to Proceed. North American Industry Classification System (NAICS) Code is 541310 with the Small Business Size Standard of 4.0 million. Construction cost estimate is between $1,000,000.00 and $2,000,000.00. Award will be made to the A/E judged by the VA to be the best qualified for the work. The following additional factors also apply: 1) Evaluation of knowledge of healthcare facilities and prior successful experience designing projects of this size, nature and cost; 2) Specific experience and qualifications of the proposed personnel assigned to the project team and record of working together as a team; 3) Capability of the firm to perform the work within required timeframes; 4) Past Performance; 5) Geographic location of the firm's office to the renovation site. Firms desiring consideration must have a working office within 600 miles of the Carl T. Hayden VA Medical Center, Phoenix, AZ 85012. All interested firms are invited to submit 254's and 255's. THIS PROJECT IS 100% SET-ASIDE FOR SMALL BUSINESS. THIS IS NOT A REQUEST FOR PROPOSALS. All interested, responsible and qualified firms within the geographical limitation desiring consideration shall submit four (4) copies of SF254's and SF255's to the attention of Donna Falcon, Contracting Officer, Carl T.
