This is a combined synopsis/ solicitation for commercial items in accordance with the format in Subpart 12.6, as supplemented with information included in this document. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a Request for Quotation (RFQ) F73HRC11781300. The solicitation document and incorporated provisions and clauses are in effect through Federal Acquisition Circular 97-27. This requirement is issued as full and open competition, as only one known source; the North American Industry Code Systems code is: 811212 (Computer and Office Machinery Repair and maintenance) the Standard Industry Code is: 7378. =20 CLIN Descriptions are as follows: CLIN 1300 -- IBML Scanner Hardware Monthly Maintenance/ Support 8x5 (1 Each) CLIN 1301 -- IBML Scanner Software Monthly Maintenance/support 8x5 (1 Each) CLIN 1302 -- DCS Scan Level III Station Maintenance (1 Each) CLIN 1303 -- DCS Smartocr Station Maintenance (3 Each OCR stations) CLIN 1304 -- DCS Coding Station Maintenance (3 Each DCS Coding Stations) CLIN 1305 -- DCS Qa Station Maintenance (1 Each)=20 CLIN 1306 -- DCS Document Split Station Maintenance (1 Each)=20 CLIN 1307 -- DCS Commit Station Maintenance (1 Each)=20 CLIN 1308 ? DCS Workgroup Server Maintenance (1 Each) CLIN 1309 ? Fujitsu Scanner Maintenance (1 Each) The following is the Statement of Work that will be required under this contract: STATEMENT OF WORK (SOW) Request the following tasks to be performed by contractor support as it pertains to the IBML scanner: A. Provide on-call (phone) maintenance support and scheduled bi-monthly on-site preventative maintenance for the following Document Conversion system hardware and software between the hours of 0800 -- 1700 Central time Monday-Friday. =20 IBML Scanner Hardware; model Imagetrac, Serial# 6603=20 IBML Scanner Software=20 Clean filters, clean all sensors, inspect and clean each Image Trac Module. Check all belts for wear, inspect all pulleys, rollers for loose set screws, clean and visually inspect all inkjet component, clean all paper, to include but not limited to dusting and debris removal from inside the onboard Image Trac PC. Clean the PC filters, check all DC power supplies with the Image Trac on and the track enabled; perform a camera adjustment (verify f-stop, set focus, adjust gain and offset and perform a set light. B. Provide on-call (phone) maintenance support for the following Document Conversion system software between the hours of 0800 -- 1700 Central time, Monday -- Friday. Also provide on-site maintenance of PC hardware and software as required to keep hardware and software operational.
