Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR, Subpart 12.12 as supplemented with additional information included in this notice. This notice constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. This solicitation is being issued as a Request for Quotation (RFQ) number 688-39-02. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular number 97-27. The NAIC code for this solicitation is 621999. The small business size standard is 500 employees. This solicitation is a small business set-aside. The contractor shal l provide labor, material, equipment, supplies, to perform a task order contract to provide all Orthosis braces and a Continuous Passive Motion Equipment covered by the contract for a period of three (3) years to include one (1) base and two (2) option years at the VA Medical Center at 50 Irving Street, NW, Washington, DC 20422. The SCHEDULE OF PRICES FOR THE BASE YEAR AND TWO OPTION YEARS shall be listed and priced as follows: The Contractor shall measure, fabricate, repair, deliver and pick up all Orthosis braces purchase by this Medical Center Prosthetic Service. The contractor shall respond within one (1) day of notification by the using service for measurement for an Orthosis (Lower Extremity or Spinal Orthosis). The measuring will be done by George Henderson, who a Board Certified Orthotist/Prothetist. Task orders will be issued by Prosthetic Service at the VAMC in Washington, DC. The contractor shall be responsible for making appointment with the beneficiaries when authorization is given by Prosthetic Service. The appointment location shall be on site (VAMC) or at the contractor office. Delivery of Custom Lower Extremity Orthosis shall be within ten (10) working days after measurement. Delivery of Spinal Orthosis shall be within three (3) working days after measurement. All finish Orthosis will be deliver to Prosthetic Clinic for check out by Dr. Owen Dillon of Orthopedic Section or Dr. Leela Chacko, Chief Rehab and Phy.Medicine. Beneficiaries will schedule for this time. The Contractor shall provide Continuous Passive Motion Equipment rental to include set-up, staff/patient education and removal for a fee charged on a daily basis to the VAMC. Delivery and set-p shall take place within 24 hours after a task order has been issued by SPD. Billing will begin upon set-up of the equipment and will stop upon notification of the end of the patient's use of the equipment The SCHEDULE OF PRICES BASE YEAR I. ORTHOSIS BRACES 1.
