Description
The Federal Bureau of Prisons, Community Corrections Contracting is seeking concerns having the ability for providing community based clinical services under Request for Quote (RFQ) 200-001-7-NC from all sources under NAICS code 621999 and the small business size standard at $9.0 million, to provide assistance to male and female Federal offenders held under the authority of the United States with medical and mental health disorders during their transition from the prison to the community. Services will be provided on-site at the Residential Reentry Center (RRC)/Comprehensive Sanctions Center (CSC) located at Cote Brilliante, St. Louis, Missouri, 63113. The services shall include but not limited to: Identification and coordination with health care providers in the area who will provide medical and mental health treatment at no cost to the inmate; Consultation with RRC/CSC staff regarding medication and treatment compliance and management of mental health disorder; Crisis intervention and support to RRC/CSC staff; Links to services to assist in gainful employment, job training, academic and vocational training (if appropriate); Education on appropriate center behavior and self management of disorder; Substance abuse treatment; Assistance with securing post release residence and community adjustment; and Assistance in obtaining SSI benefits as indicated by the inmate needs. The service provider will provide clinical case management services performed by a clinical licensed social worker in the State of Missouri or an individual with an equivalent degree and licensure in the State of Missouri in Social Services. The service provider will be responsible for all clinical case management functions relative to the medical and mental health needs of the inmates in the RRC/CSC, and serve as the subject matter expert for mental health issues that arise involving these inmates. Services to be provided includes (a) Assessment and Development of service plan, (b) Coordination and monitoring of service plan, and (c) Intervention services. The initial assessment will conducted at the RRC/CSC facility. One hour is equal to one unit. Performance period and requirements under this contract will be: 1. Base Year - 11/01/2007 - 10/31/2008: (a) Assessment and development of service plan (25 inmates X 2 units per inmate) = 50 units; (b) Coordination and monitoring of service plan (25 inmates X 4 units per inmate) = 100 units; ( c) Intervention services (25 inmates X 1 unit per inmate) = 25 units 2. Option Year 1 - 11/01/2008 - 10/31/2009: (a) Assessment and development of service plan (25 inmates X 2 units per inmate) = 50 units; (b) Coordination and monitoring of service plan (25 inmates X 4 units per inmate) = 100 units; ( c) Intervention services (25 inmates X 1 unit per inmate) = 25 units 3. Option Year 2 - 11/01/2009 - 10/31/2010: (a) Assessment and development of service plan (25 inmates X 2 units per inmate) = 50 units; (b) Coordination and monitoring of service plan (25 inmates X 4 units per inmate) = 100 units; ( c) Intervention services (25 inmates X 1 unit per inmate) = 25 units 4. Option Year 3 - 11/01/2010 - 10/31/2011: (a) Assessment and development of service plan (25 inmates X 2 units per inmate) = 50 units; (b) Coordination and monitoring of service plan (25 inmates X 4 units per inmate) = 100 units; ( c) Intervention services (25 inmates X 1 unit per inmate) = 25 units 5. Option Year 4 - 11/01/20011 - 10/31/2012: (a) Assessment and development of service plan (25 inmates X 2 units per inmate) = 50 units; (b) Coordination and monitoring of service plan (25 inmates X 4 units per inmate) = 100 units; ( c) Intervention services (25 inmates X 1 unit per inmate) = 25 units The services required above shall be in accordance with the Federal Bureau of Prisons, Statement of Work (SOW) Community Based Clinical Services (Adult Mental Health) dated June 2007. The solicitation will be available on or before July 10, 2007 and will be distributed solely through the General Services Administration?s website http://www.fbo.gov/. Hard copies of the solicitation document will not be available. The site provides downloading instructions.
