- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is W91YTV-10-T-0035. This acquisition is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-38. The North American Industry Classification System Code (NAICS) for this procurement is: 339113; the Standard Industrial Classification Code is 3841; and the small business size standard is 500. This requirement is (UNRESTRICTED). Blanchfield Army Community Hospital, Fort Campbell, KY has a requirement for the following: Item No. 0001: Establish a Blanket Purchase Agreement to provide all labor, material, equipment and transportation necessary to medical equipment to Orthotripsy Equipment as required by the Government. The equipment will be used perform non-invasive surgical procedures (bilateral tendonopathy or unilateral tendonopathy), for Blanchfield Army Community Hospital, Fort Campbell, KY. The contractor shall ensure the equipment is on site on the day before the procedures are to be performed in order for Medical Equipment Maintenance to inspect/certify the equipment. Payment will be made by 25K Purchase Card. The period of performance will be from 01 February 2010 through 30 September 2014. The Health Insurance Portability And Accountability Act Of 1996, Privacy And Security Of Protected Health Information (Mar 2008) applies to this action. THE SOLICITATION MUST BE QUOTED ALL OR NONE.
