- PERFORMANCE WORK STATEMENT FOR Health Care Delivery for the 509th MDG, Whiteman AFB, MO SOW for Medical Technician for Medical Material Logistics (NON PERSONAL SERVICE) SECTION TITLE 1 DESCRIPTION OF SERVICES 2 SERVICE DELIVERY SUMMARY 3 GOVERNMENT AND CONTRACTOR FURNISHED PROPERTY AND SERVICES 4 Specific Task Performance Work Statement GENERAL 1.1. Scope of Work. 1.1.1. The Contractor shall provide a for Medical Logistics Technician to work at the, Whiteman AFB, Missouri clinic, also referred to as the "Medical Treatment Facility" (MTF) herein. The Medical Logistics Tech will begin performing services from 16 Feb 29 thru Nov 29, 2010. The Medical Technician for Medical Logistics must have a favorable clearance and be able to get a Flight line badge. 1.1.2. Contractor shall furnish all labor for the position of Medical Logistics Technician services provided for a medical treatment facility. Performance shall be according to the requirements contained in this performance work statement (PWS). 1.1.3. Contract personnel are subject to the rules and regulations of the MTF on: telephonic availability, The MTF has the right to adjust the above list as rules and regulations are updated. Contract supplied personnel will park in staff parking areas.. Duties and responsibilities of contract personnel are defined by the MTF. NOTE: The exact amount (i.e.# of FTEs) of Resources that will be provided under this contract shall be mutually agreed to by the Contractor and the MTF. The MTF also reserves the option to designate which employee will be considered "Mission Essential" (for inclement weather, contingency operations and exercises, etc.). 1.2. PERSONNEL. 1.2.1. POINT OF CONTACT. The Contractor shall provide a point of contact that shall be responsible for the performance of the work. The point of contact shall have full authority to act for the Contractor on all matters relating to the operation of this contract. The Contractor shall designate this individual, in writing. 1.2.2. AVAILABILITY. The point of contact shall be available via telephone five (5) days a week, Monday through Friday, 7:30 AM until 4:30 PM, excluding Federal Holidays (defined herein as a "duty day"). 1.2.3. ARRANGE FOR REPLACEMENT STAFF. The contractor shall arrange for coverage when contract personnel will be unable to provide services for 10 (ten) or more consecutive scheduled shifts. Replacement Staff must meet the same requirements. 1.2.4. WORK ROSTER. The contractor's point of contact shall provide a list by name, start date, and social security # of individual personnel the Contractor shall use to provide services. This list shall be provided 30 days in advance. The Contractor shall submit changes in writing to the QAP at least 15 days in advance of the proposed change. 1.2.5. PERSONNEL REQUIREMENTS. 1.2.5.1. ENGLISH LANGUAGE REQUIREMENT. Personnel shall read, understand, speak, and write English well enough to effectively communicate with patients and other HCWs. 1.2.5.2. APPEARANCE: Contractor personnel shall present a neat appearance and be easily recognized as contractor employees. A Medical Group issued nametag will be provided designating "contractor". 1.2.5.3. ATTENDANCE AT MEETINGS. Required personnel shall attend and participate in meetings, professional staff conferences, and other appropriate professional activities as directed by the Government Manager or Element Chief. 1.2.5.4. CONTINUING EDUCATION : Contractor employees who require continuing education to maintain their license/credentials/certifications are responsible to acquire them on their own without disruption of services and at no expense to the government. 1.2.6. CRIMINAL BACKGROUND CHECK REQUIREMENT. 1.2.6.1. The Contractor will conduct National Agency Check criminal background checks on all personnel providing services under this contract using the automated SF85P, meeting the formal requirements for the criminal background checks. Background checks must be completed 30 days prior to personnel starting work in the MTF. Evidence of such submission must be included in the initial credentialing packets. 1.2.6.2. With the written approval of the MTF Security Manager, contract personnel may provide contract services prior to completion of background checks. However, at all times while children are in the care of that individual, the contract personnel shall be within sight and continuous supervision of a staff person, whose background check has been completed, a chaperone, or parent/guardian. 1.2.6.3. Contract employees shall have the right to obtain a copy of any background check pertaining to them and to challenge the accuracy and completeness of the information contained in the report. 1.2.6.4. Contract employees who have previously received a background check shall provide to the Contract Manager upon start-up of employment proof of the check or obtain a new one. 1.2.7. CONFLICT OF INTEREST. The Contractor shall not employ any person who is an employee of the United States Government if the employment of that person would create a conflict of interest. The Contractor shall not employ any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives approval in accordance with DoD Directive 5500.7 and Air Force policy. 1.2.8. REQUIRMENTS a. Copy of valid, current, certifications and or experience. b. Personnel education and training. c. Current Basic Life Support (BLS). Must be current in the completion of American Heart Association Heart Saver CPR with AED or American Red Cross equivalent. The Government will provide refresher training. g. A copy of the proposed contract personnel's professional resume, accompanied by that individual's sworn affidavit of the truthfulness of same, indicating experience, training, and technical expertise in the type of work to be performed. h. A copy of all certifications/licences in which contract personnel currently hold or have held a license to the related services. i. Certification of current physical examination for each employee. The certification shall contain a signed statement by the examining physician stating that the employee is free of any contagious diseases. Physical examination must be current and within the 12 months prior to the start of employment. 1.3.3. The Contractor is required to begin full performance of the contract requirements by the commencement date of the contract. 1.4. MEDICAL LIABILITY INSURANCE. The Contractor shall provide and maintain adequate liability coverage consistent with the risks associated with the performance of all services required by this PWS. Refer to Federal Acquisition Regulation (FAR). Insurance requirements must meet minimum standards as noted in an offeror's base FSS contract. 1.6. EDUCATION AND TRAINING REQUIREMENTS 1.6.2. CERTIFICATION. Any certifications related to Medical Logistics 1.6.3. EXPERIENCE. Previous Medical Logistics experience 1.7. EDUCATION AND TRAINING REQUIREMENTS High School education 1.7.1. Must be current in certification of Basic Life Support with completion of American Heart Association HeartSaver CPR with AED or American Red Cross equivalent. The MTF will provider refresher training only. 1.7.2. LICENSE/REGISTRATION. None 1.8.2.0. EVALUATIONS. The contractor shall conduct reviews of all personnel within 60 days of the onset of contract services and after 12 months of service. Annually thereafter. 1.9. HEALTH REQUIREMENTS: 2.0. PREASSIGNMENT MEDICAL EVALUATION: No contract employee shall perform health care services under this contract unless a pre-assignment medical evaluation has been performed not more than 30 calendar days prior to submission of credentials. The purpose of this medical evaluation is to determine if the individual, from a medical standpoint, possesses the minimum physical abilities needed to perform the proposed job without significant risk to personal health or the health and safety of others. As a minimum, the individual shall be free of physical defects or medical conditions which might reasonably be expected to place other workers, patients, or the public at risk. Not later than five (5) days prior to the commencement of performance of health care services under this contract by any employee, the Contractor shall provide to the Public Health (Employee Health) Flight certification that such employees have completed the medical evaluation requirements identified below. This certification shall state the date on which the examination was completed, the name of the doctor who performed the examination and a statement concerning the physical health of the individual. Initial documentation is required for: MMR (immunization or titer levels for Rubella and Rubeola), history of chicken pox disease or positive titer (varicella vaccination must be accomplished if lack of immunity), TB Mantoux skin test (IPPD); if positive, minimum requirement will be follow-up with contractor's physician and a chest x-ray. A physician must provide documentation of "no active respiratory disease", Hepatitis B vaccination/series are required for all personnel with occupational risk of exposure (declination is not acceptable, this is a condition of employment) and if the employee will be involved in exposure-prone procedures (ex: dentist, dental hygienist) they will have their HbsAg status determined. This is to be done by providing documentation of the HbsAb (if positive they will be referred to the MTF Credentials committee to determine scope of practice). If, in the opinion of the evaluating medical practitioner, the individual is found to be qualified for duty, the medical statement shall include the following wording: "(NAME OF EMPLOYEE) possesses the minimum physical abilities to perform the proposed duties of a (insert job title). He/she has documented proof of immunization against/immunity to rubeola, rubella, and hepatitis B and does not have an active infectious condition which might place others at risk." If, in the opinion of the evaluating medical practitioner, the individual does not possess the minimum capabilities needed to perform the proposed job or does not meet the conditions of employment or will pose a risk to others, the medical statement shall contain the following wording: "For medical reasons, (NAME OF EMPLOYEE) is not qualified for employment as a (insert job title)." Certification shall be provided to the CO, who will forward to the MTF Employee Health Office. 2.1 ANNUAL MEDICAL EVALUATION. All contract employees performing health care services under this contract shall complete annual medical evaluations as required by the MTF hospital employee health program. The contractor shall provide to the QAP a certificate, as described above, documenting the results of this evaluation. 2.2 PREVENTATIVE, PROPHYLACTIC, AND FOLLOW-UP PROCEDURES. The contractor shall ensure that his/her employees are in compliance with preventive, prophylactic and follow-up procedures, as well as infection control and employee health program procedures, as established by the MTF. Required preventive, prophylactic and follow-up procedures will be provided by the Government to contract employees in the military MTF. The Contractor shall reimburse payment for these services. If care is received elsewhere, the Contractor shall provide written verification of treatment. 2.3. PREGNANT EMPLOYEES. Contract employees should report their pregnancy to the contractor. It is the responsibility of the contractor to notify the QAP of the pregnancy. MTF Employee Health Office will provide information concerning any work hazards in that area. The contractor is to notify their pregnant employee of any work hazards. It will be the employee and the contractor's joint decision whether the contracted employee works in the environment. 2.4. EMERGENCY HEALTH CARE. The MTF will provide emergency health care for contract personnel for injuries occurring while on duty in the MTF. These services will be billed to the Contractor at the current full reimbursement rate. 2.5. HOURS OF OPERATION. The contract employee shall work a minimum of 40 hours per week, unless specified by the MTF. Duty hours may consist of typical clinic hours, usually 7:00 AM - 4:30 PM, Monday thru Friday. Policy regarding daily lunch breaks and other breaks will follow the government's standard procedures for contract personnel. The contract personnel should follow command announcements for reporting to work as it relates to inclement weather or base closures. 2.6. FEDERAL HOLIDAYS. Contract personnel will not be required to provide service on recognized Federal Holidays, Wing down days and ACC training Days. The following are recognized Federal Holidays(New Years Day, Martin Luther King, JR's Birthday, Presidents' Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, Christmas Day) and if the president names another day a holiday. 2.7. SECTION FLEXIBILITY. Contract personnel may be required to work in other MTF duty sections. Contract personnel shall be flexible in rotating temporarily, or permanently moving into other duty sections upon notification of the Flight Chief. 2.8. MEDICAL QUALITY ASSURANCE/RISK MANAGEMENT (QA/RM). 2.9. Contract personnel shall participate in QA/RM activities to the extent required by AFI 44-119 and the individual MTF QA/RM plan or regulation. 2.10. The Government will evaluate the personnel's professional, as differentiated from administrative, performance under this contract using quality assurance standards specified in AFI 44-119. Nothing in this paragraph precludes the Government from also conducting inspections under the Inspection of Services clause (FAR 52.246-4). 2.11 ORIENTATION. The Contractor shall ensure that all contract providers participate in the MTF orientation and service/clinic specific orientation prior to furnishing services under this contract. Clinic orientation must occur within five duty days of employment start date. Medical Center orientation must occur within 30 days of employment start date. 2.12 CONTINUING EDUCATION REQUIREMENTS. Continuing Education shall be obtained at no additional cost to the Government. 2.13. ANNUAL MDG SPECIFIC TRAINING REQUIREMENTS: All contract employees must complete annual MTF training required IAW HIPAA, AF Computer Security Policy, and AF Safety Policy. 2.14. GOVERNMENT COMPUTER ACCESS: The contractor will ensure that all contract employees comply with the requirement to obtain the minimum personnel security investigations as prescribed by DoD 5200.2-R and AFI 31-501, paragraphs 3.27.3.7 to 3.27.3.7.2. The contractor will work with the MTF to ensure that the pre-employment screening process for each employee classified as ADP I, II, III (as defined in DoD 5200.2-R) includes the appropriate investigations and that each contract employee has the appropriate questionnaires and forms to be completed. All completed forms will be reviewed by the contractor and forwarded to the MTF for processing prior to the employee beginning work. The contractor understands that, while the MTF Commander may allow contractor personnel to temporarily occupy non-critical sensitive positions pending National Agency Check (NAC), the employee will be immediately removed from the position if at any time the NAC receives unfavorable adjudication, or if other unfavorable information that would affect the NAC becomes known. This must be completed 30 days prior to the start of heath care delivery. 2.15 IDENTIFICATION BADGES. IAW 509th Medical Group Instruction 36-102, the wear of a government provided photo identification badge is required by all contract personnel. 2.16. CUSTOMER COMPLAINTS. Any customer complaints will be documented on the AF Form 714, Customer Complaint Record. Governement Computer Access 2.14 All documentation has been submitted for approved access to government computer systems. 100% Compliance SECTION 3 GOVERNMENT/CONTRACTOR FURNISHED PROPERTY AND SERVICES 1.1 CONTRACTOR FURNISHED ITEMS AND SERVICES 1.2. GENERAL. Except for those items or services specifically stated in Section III to be Government furnished, the Contractor shall furnish everything required to perform this contract in accordance with all of its terms. 1.3. GOVERNMENT FURNISHED ITEMS AND SERVICES 1.4. GENERAL. The Government will provide the following equipment, supplies, and services listed below: 1.5. EQUIPMENT/OFFICE FURNITURE. The personnel shall have joint use of all available equipment for performing services required by this contract. 1.6. PAGER. NA 1.7. PERSONAL PROTECTIVE EQUIPMENT (PPE). The Government will furnish the contract personnel with appropriate PPE. The Government will be responsible for any repair, cleaning, and inventory required for the PPE. This does not include any type of uniform or laboratory coat. 1.8. FORMS. The MTF will provide required government forms used in the performance of services. 1.9. SUPPLIES. The MTF will provide medical and non-medical supplies commonly used in the facility for the care and management of patients. 1.10. COMPUTER EQUIPMENT. The MTF will provide computer equipment required. The MTF will provide required training for these systems. The contractor will be required to use the computer systems that are standard for the support of the MTF. SECTION 4 SPECIFIC TASKS SUMMARY: This position will serve as a Supply Technician providing operations support pertaining to medical supply items. The duties will include forward logistics, receiving, stocking, selecting and storing medical supply items, supply turn-in, and medical group vehicle care taking. This includes receiving, storing, selecting, shipping, pick-up and delivery of general or specialized bulk materials and equipment. 1 QUALIFICATIONS. 1.1 Knowledge and skills. 1.1.1 Knowledge of governing supply regulations, procedures and instructions. 1.1.2 Knowledge of supply functions, operations, program requirements and work methods. 1.1.3 Knowledge of warehousing methods, procedures, material handling techniques and of activities such as material receiving, issuing and preservation. 1.1.4 Ability to use automated processing equipment and systems such as optical readers and scanners. 1.1.5 Ability to read, interpret and apply reference material such as regulations, manuals, catalogs, records, orders, etc. to accomplish and resolve problems. 1.1.6 Ability to read and speak the English language, correct grammar, spelling, punctuation, capitalization, and format to prepare and edit written correspondence, reports, and transcribed material. 1.1.7 Standard office equipment, such as computers, typewriters, copiers, fax machines, telephone systems and office automation systems, to perform a substantial range of office support. 1.1.8 General medical ethics, telephone etiquette, excellent communications and customer service skills. 1.2 Education/Certification. 1.2.1 Must have a High School Diploma and the ability to learn new tasks. 1.3 Experience. A minimum of 6 months experience is required in the supply or warehouse setting. 1.4 Physical Requirements. Work is a combination of sedentary duties as well as light to moderate physical effort. Requirements include walking, standing, reaching, bending and moving items. May require lifting and carrying weight over 45 pounds. Works periodically on hard surfaces. 1.5 Work Environment. Work is performed indoors in an environment involving everyday risks or discomforts which require normal safety precautions. The area is adequately lighted, heated and ventilated. 1.6 Other Considerations. 1.6.1 May serve to train/guide lower grade (or other trades) military/civilian employees. 1.6.2 The contractor may require driving privileges and be able to drive up to a 1.5 ton stake bed truck and 4-6K electric forklift for the delivery of medical equipment, supplies, and furniture. 2.0. Communication Skills: 2.1 Demonstrate strong customer service skills and foster development of those skills throughout entire team. 2.2 INFECTION CONTROL AND SAFETY. The contractor shall comply with all MTF infection control and safety procedures, practices, and standards. 2.3. HEALTH SERVICES INSPECTION (HSI). The Contractor shall meet all applicable HSI standards during the performance of this contract. 2.4. RECORDS. If requested by the Government, the Contractor shall provide the original record, or a reproducible copy of any such records within five working days of receipt of the request. 2.5. RELEASE OF MEDICAL INFORMATION. The personnel shall only release medical information obtained during the course of this contract to other MTF staff involved in the care and treatment of that individual patient. Patient lists, no matter how developed, shall be treated as privileged information. Lists and/or names of patients shall not be disclosed to or revealed in anyway for any use outside the MTF without prior written permission by the Chief of Hospital Services. All patient information will comply with HIPAA standards. 2.6. HEALTH INSURANCE PORTABILITY AND ACCOUNTABILITY ACT (HIPAA) OF 1996: All individuals performing services at the 509th Medical Group are required to complete the TMA provided "online HIPAA training", training on local MTF HIPAA policies, and will be held accountable for complying with 509th Medical Group MDGI's regarding health information privacy. PRIVACY OF PROTECTED HEALTH INFORMATION (a) Definitions. As used in this clause: Individual has the same meaning as the term ``individual'' in 45 CFR 164.501 and shall include a person who qualifies as a personal representative in accordance with 45 CFR 164.502(g). Privacy Rule means the Standards for Privacy of Individually Identifiable Health Information at 45 CFR part 160 and part 164, subparts A and E. Protected Health Information has the same meaning as the term ``protected health information'' in 45 CFR 164.501, limited to the information created or received by The Contractor from or on behalf of The Government. Required by Law has the same meaning as the term ``required by law'' in 45 CFR 164.501. Secretary means the Secretary of the Department of Health and Human Services or his/her designee. Terms used, but not otherwise defined, in this Agreement shall have the same meaning as those terms in 45 CFR 160.103 and 164.501. (b) The Contractor agrees to not use or further disclose Protected Health Information other than as permitted or required by the Contract or as Required by Law. (c) The Contractor agrees to use appropriate safeguards to prevent use or disclosure of the Protected Health Information other than as provided for by this Contract. (d) The Contractor agrees to mitigate, to the extent practicable, any harmful effect that is known to the Contractor of a use or disclosure of Protected Health Information by the Contractor in violation of the requirements of this Contract. (e) The Contractor agrees to report to the Government any use or disclosure of the Protected Health Information not provided for by this Contract. (f) The Contractor agrees to ensure that any agent, including a subcontractor, to whom it provides Protected Health Information received from, or created or received by the Contractor on behalf of the Government agrees to the same restrictions and conditions that apply through this Contract to the Contractor with respect to such information. (g) The Contractor agrees to provide access, at the request of the Government, and in the time and manner designated by the Government to Protected Health Information in a Designated Record Set, to the Government or, as directed by the Government, to an Individual in order to meet the requirements under 45 CFR 164.524. (h) The Contractor agrees to make any amendment(s) to Protected Health Information in a Designated Record Set that the Government directs or agrees to pursuant to 45 CFR 164.526 at the request of the Government or an Individual, and in the time and manner designated by the Government. (i) The Contractor agrees to make internal practices, books, and records relating to the use and disclosure of Protected Health Information received from, or created or received by the Contractor on behalf of, the Government, available to the Government, or at the request of the Government to the Secretary, in a time and manner designated by the Government or the Secretary, for purposes of the Secretary determining the Government's compliance with the Privacy Rule. (j) The Contractor agrees to document such disclosures of Protected Health Information and information related to such disclosures as would be required for the Government to respond to a request by an Individual for an accounting of disclosures of Protected Health Information in accordance with 45 CFR 164.528. (k) The Contractor agrees to provide to the Government or an Individual, in time and manner designated by the Government, information collected in accordance with this Clause of the Contract, to permit the Government to respond to a request by an Individual for an accounting of disclosures of Protected Health Information in accordance with 45 CFR 164.528. General Use and Disclosure Provisions Except as otherwise limited in this Agreement, the Contractor may use or disclose Protected Health Information on behalf of, or to provide services to, the Government for the following purposes, if such use or disclosure of Protected Health Information would not violate the Privacy Rule or the Department of Defense Health Information Privacy Regulation if done by the Government: [All purposes specified in Department of Defense Regulation 6025.18-R, Defense Health Privacy Regulation]. Specific Use and Disclosure Provisions (a) Except as otherwise limited in this Agreement, the Contractor may use Protected Health Information for the proper management and administration of the Contractor or to carry out the legal responsibilities of the Contractor. (b) Except as otherwise limited in this Agreement, the Contractor may disclose Protected Health Information for the proper management and administration of the Contractor, provided that disclosures are required by law, or the Contractor obtains reasonable assurances from the person to whom the information is disclosed that it will remain confidential and used or further disclosed only as required by law or for the purpose for which it was disclosed to the person, and the person notifies the Contractor of any instances of which it is aware in which the confidentiality of the information has been breached. (c) Except as otherwise limited in this Agreement, the Contractor may use Protected Health Information to provide Data Aggregation services to the Government as permitted by 45 CFR 164.504(e)(2)(i)(B). (d) Contractor may use Protected Health Information to report violations of law to appropriate Federal and State authorities, consistent with 45 CFR 164.502(j)(1). Obligations of the Government Provisions for the Government to Inform the Contractor of Privacy Practices and Restrictions (a) Upon request the Government shall provide the Contractor with the notice of privacy practices that the Government produces in accordance with 45 CFR 164.520, as well as any changes to such notice. (b) The Government shall provide the Contractor with any changes in, or revocation of, permission by Individual to use or disclose Protected Health Information, if such changes affect the Contractor's permitted or required uses and disclosures. (c) The Government shall notify the Contractor of any restriction to the use or disclosure of Protected Health Information that the Government has agreed to in accordance with 45 CFR 164.522. Permissible Requests by the Government The Government shall not request the Contractor to use or disclose Protected Health Information in any manner that would not be permissible under the Privacy Rule if done by the Government, except for providing Data Aggregation services to the Government and for management and administrative activities of the Contractor as otherwise permitted by this clause. Termination (a) Termination. A breach by the Contractor of this clause, may subject the Contractor to termination under any applicable default or termination provision of this Contract. (b) Effect of Termination. (1) If this contract has records management requirements, the records subject to the Clause should be handled in accordance with the records management requirements. If this contract does not have records management requirements, the records should be handled in accordance with paragraphs (2) and (3) below (2) If this contract does not have records management requirements, except as provided in paragraph (3) of this section, upon termination of this Contract, for any reason, the Contractor shall return or destroy all Protected Health Information received from the Government, or created or received by the Contractor on behalf of the Government. This provision shall apply to Protected Health Information that is in the possession of subcontractors or agents of the Contractor. The Contractor shall retain no copies of the Protected Health Information. 3) If this contract does not have records management provisions and the Contractor determines that returning or destroying the Protected Health Information is infeasible, the Contractor shall provide to the Government notification of the conditions that make return or destruction infeasible. Upon mutual agreement of the Government and the Contractor that return or destruction of Protected Health Information is infeasible, the Contractor shall extend the protections of this Contract to such Protected Health Information and limit further uses and disclosures of such Protected Health Information to those purposes that make the return or destruction infeasible, for so long as the Contractor maintains such Protected Health Information. Miscellaneous (a) Regulatory References. A reference in this Clause to a section in the Privacy Rule means the section as in effect or as amended, and for which compliance is required. (b) Survival. The respective rights and obligations of Business Associate under the ``Effect of Termination'' provision of this Clause shall survive the termination of this Contract. (c) Interpretation. Any ambiguity in this Clause shall be resolved in favor of a meaning that permits the Government to comply with the Privacy Rule. 3.0. COMPUTER SECURITY: A National Agency Check must occur for clearance for appropriate security of privacy act information. Contractor is responsible for completing SF85P Worksheet and providing fingerprint cards for each employee immediately with the hiring process, complying with regulations, DoD 5200.2-R, Appendix K and AFSSSI 5027, Section 5.3.3. The above must be completed and submitted to the MTF 30 days prior to personnel starting work. 4.0. COMMUNICATION. The Contractor shall ensure contract personnel maintain open and professional communication with MTF personnel. Complaints validated by the QAP and Chief of the Medical Staff shall be reported in writing to the contract administrator and the Contractor for action. Failure of the Contractor to correct validated complaints raised by the MTF staff and the CO will be considered a failure to perform. 5.0. PERFORMANCE EVALUATION MEETINGS. The Contractor or their representative is to meet with the designated MTF representative and other government personnel as often as deemed necessary. The Contractor may request a meeting when he/she deems such necessary. Meetings will be documented in the contract file with written minutes signed by the contract manager and the or contract administrator. Should the Contractor not concur with the minutes, such non-concurrence shall be provided in writing to the Contract Specialist within ten (10) calendar days of receipt of the minutes. 6.0. DOCUMENTATION. All contract personnel shall prepare all documentation to meet or exceed established standards of the MTF to include but not limited to: timeliness, legibility, accuracy, content and signature. Only MTF and Air Force approved abbreviations may be used to document care in the health care record. Contract personnel shall ensure complete patient identifying information is on all documentation that is to become part of the health care record. Corrections will be made by the Government or under the direction of the government representative. 7.0. OTHER SUPPORT. Contract personnel shall provide immediate evaluation and management of emergency problems as they occur. 8.0. REFERRALS AND CONSULTS. Contract personnel shall follow DoD, Air Force and MTF regulations and policies when arranging for a referral or consult. 9.0. PERSONNEL/SUPPORT STAFF REIMBURSEMENT FOR HEALTHCARE DELIVERY.
