- INTENT TO SOLE SOURCE The Department of Veterans Affairs, Network Contracting Office 01 (NCO1) intends to sole source a contract for a Lumenis Selecta Trio All-in-One Laser System at the Togus, Maine VAMC. All work is to be completed in accordance with statement of work and schedule. This sole source contract will be solicited and negotiated with OEM SDVOSB distributor Melling Medical The North American Industry Classification System (NAICS) code for this procurement is 339112-Surgical and Medical Instrument manufacturing. The Small Business Administration (SBA) size standard is 1000. Interested vendors that can meet this requirement, should identify their interest and capability, and respond to this notice by 02/14/2022 by 2:00 PM EST. Interested vendors must send a capability statement no more than one page in length; to include Company Name, Duns Number, Socioeconomic Category, brief description of your business entities capability to handle this particular requirement, website and GSA Contract number (if applicable) and/or other VA Medical Center past contract information. No solicitation will be made available. This is not a request for quotations. All information shall be provided at no cost or obligation to the Government. Responses must be sent via email to Contract Officer, Deborah. Reardon @va.gov. If no responses are received, Contracting will proceed with the sole source negotiation with Melling Medical. Performance Work Statement (PWS) Design and Content 1.0 Introduction The VA Maine HCS requires One Lumenis Selecta Trio All-in-one laser system for the Eye clinic at Togus. 2.0 Background The lumenis Selecta Trio is the gold standard as it is the comprehensive solution for the ophthalmic practice, providing glaucoma, cataract, and retinal therapy in a single platform. Lumenis Selecta Trio combines the advantages of Selective Laser Trabeculoplasty (SLT), YAG and photocoagulation, all developed and brought to market by Lumenis. The Lumenis laser is currently utilized in hundreds of VA Medical Centers and Department of Defense facilities across the country and upholds to VA directives and policies. Scope (Provide an overview of the PWS emphasizing the most important aspects of your requirement and defining the boundaries of the contractor s performance responsibilities). This project will provide one State of the Art Lumenis Selecta Trio All-in-one laser for the Eye Clinic department. The Lumenis TRIO is an advanced design combination laser that sets numerous technological and clinic gold standards. The equipment and manufacturer are well known to our clinic. This equipment is integral to delivery of services and considered the standard of care. They have provided our clinic with numerous instruments that have successfully been used to improve the health and well-being of our Veteran population for the past several years. Lumenis is a recognized manufacturer of the highest quality medical lasers that provide the highest level of safety based on OSHA and ANSI standards. The equipment is constructed using only the highest quality materials (e.g., housing, locks, filters) in such a manner that risk of accident and adverse event are minimized. The equipment includes also includes personal protective equipment and laser signage to promote a safe environment. This is the standard of care outside the VA and with essential features as. Homogeneous Laser Beam- Our advanced optics and laser delivery technology features uniform energy distribution across the entire laser spot area for more precise and safer photocoagulation treatment. The result is a homogenous laser spot with evenly distributed power across the full area of the spot, eliminating the potential risk for the formation of hot spots. Dual Fiber Port Instantly switch between delivery devices. Changing delivery devices is fast and easy. The Novus Spectra automatically senses the delivery devices connected to it. Laser parameters are adjusted internally to compensate for the chosen delivery device to maintain selected power density on the target tissue. Clear View Filters Optimized view of treatment site. The Lumenis Clear View filters photopic balanced coating eliminates color distortion of the treatment site while optimizing white light transmission or brightness optimizing light transmission. The Selecta Duet and or Trio have a built in Physician Eye Safety Filter, that protect the user from both the YAG and Photocoagulation laser wavelengths. AcuGuide Micromanipulator-Accurate and safe laser beam guidance for fine-tuned laser application on target tissue. Integrated Laser Link Cradle- Safely and conveniently stow the laser adapter when not performing photocoagulation. Q-Switched frequency - doubled 532 nm laser for selective laser trabeculoplasty, the choice of therapy for minimally invasive treatment of primary open-angle glaucoma 1064 nm Nd: Yag - the essential photo disruptor for point and shoot capsulotomy and iridotomy Laser beam spot size 400um at visual focal plane Wheelchair accessible, travel 250mm (10.4 in) S-link (GA-0020310) Selecta 532 nm fixed ESF Laser Link for New Selecta Zeiss style laser link 50 1000-micron spot size telescope Micromanipulator, 532 Mn fixed ESF, SMA 906, and Compatible with New Selecta Family of Lasers only 4.0 Applicable Directives (List all documents referenced in the next section 5.0, Performance Requirements, with which the contractor will have to comply in performance of the contract effort. The system must meet the manufacturer s specifications and be designed and implemented within the vendors 510K system design as an FDA regulated medical device/system. 5.0 Performance Requirements (Identify what the contractor is required to do (major tasks and subtasks). The vendor will as part of their project plan outline a timeline for implementing the project for the Eye Clinic at Togus, VA Maine He6.0 Deliverables (List all data required to manage or monitor contractor performance). This project will include all and any peripheral items to ensure a fully functional system such as one pair of safety spectacles, operator manual and laser safety sign. Selecta Latina Gonio Lens Designed specifically for Selective Laser Trabeculoplasty. 1.0X magnification maintains laser spot size and 1 to 1 laser energy delivery. Tilted anterior lens surface corrects astigmatism to maintain circular laser beam profile and give sharp images for examination. Suitable for standard laser trabeculoplasty. The total quote includes Warranty, Shipping and Handling, On-site training, and Installation Warranty: Three-year standard warranty on parts and labor Lumenis warrants that the Equipment (excluding consumable or disposable components or accessories such as heads and fibers, which will be subject to the warranty terms, if any, included therewith) will be free from defects in materials and workmanship and will conform in all material respects to Lumenis technical specifications. The foregoing limited warranty shall continue for a period which is the shorter of (a) fifteen (15) months from the date of Buyer s receipt of the Equipment or (b) thirty-six (36) months from the date of installation if the Equipment .
