Description
THIS REQUEST FOR INFORMATION (RFI) 36C26222Q0389 -IS ISSUED SOLELY FOR INFORMATION AND PLANNING PURPOSES ONLY AND DOES NOT CONSTITUTE A SOLICITATION. THE SUBMISSION OF PRICING, CAPABILITIES FOR PLANNING PURPOSES, AND OTHER MARKET INFORMATION IS HIGHLY ENCOURAGED AND ALLOWED UNDER THIS RFI IN ACCORDANCE WITH (IAW) FAR 15.201(e). SOURCES SOUGHT DESCRIPTION This is NOT a solicitation announcement. This is a source sought/RFI for market research only. The purpose of this source sought/RFI is to gain knowledge of potential qualified sources and their size classification relative to NAICS 339113 (size standard is 750 Employees). Responses to this source sought will be used by the Government to make appropriate acquisition decisions. After review of the responses to this source sought, a solicitation announcement may be published. Responses to this source sought synopsis are not considered adequate responses for a solicitation announcement. The Department of Veterans Affairs (VA), VISN 22 Network Contracting Office, is seeking sources that can provide Procedure Chairs that at a minimum meets the following salient characteristics in the Description of Equipment for the VA Loma Linda Healthcare System: LINE ITEM DESCRIPTION QUANTITY UNIT Unit Price Amount 0001 Midmark 630 Humanform Procedure Chair, Programmable with Rotation base, or equal to 8 EA     0002 Upholstery Top (including headrest and footrest), 28 wide, Premium, Undecided Color, or equal to 8 EA     0003 Chair Arms, Upholstery, Undecided Color, or equal to 8 EA     TOTAL   Include shipping cost on the items price. Vendor Requirements: Period of Performance is 60 days after receipt of order (ARO). Vendor shall quote all items under this requirement. Vendor shall meet the Salient Characteristics in the Description of Equipment. Vendor shall respond questions 1 through 8 of the Request of Information (RFI), see below. DESCRIPTION OF EQUIPMENT FOR VA LOMA LINDA HEALTHCARE SYSTEM Objective 1.1. The contractor shall provide to the VA Loma Linda Healthcare System upholstered procedure chairs (with upholstered chair arms) that can accommodate bariatric patients: the Midmark 630 Humanform Procedure Chair with Chair Arms and Upholstery Top (including headrest and footrest) or equal to. Background 2.1. This is a Firm Fixed Priced contract. Product Description and Quantities LINE ITEM DESCRIPTION QUANTITY UNIT 0001 Midmark 630 Humanform Procedure Chair, Programmable with Rotation base, or equal to 8 EA 0002 Upholstery Top (including headrest and footrest), 28 wide, Premium, Undecided Color, or equal to 8 EA 0003 Chair Arms, Upholstery, Undecided Color, or equal to 8 EA Pictures provided strictly as reference for vendors for guidance. Period of Performance 4.1. Period of Performance is 60 days after receipt of order (ARO). Place of Delivery 5.1. VA Loma Linda Healthcare System 11201 Benton Street Warehouse 1C-05 Loma Linda, CA 92354 Salient Characteristics Chair Rotational Base Capacity of 500 lbs. or more to accommodate routinely admitted bariatric patients experiencing chronic pain. Easy positioning capability to safely transfer patients and/or to safely reposition patients in upright, prone, or supine positions during examination of chronic conditions. Rotational base to effectively and safely position patients in properly examining, diagnosing, and treating their chronic conditions within the limitations of available room space capacity. Brake for rotation to examine the patient safely and effectively. Functionality that automatically stops chair movement when potential harmful impact is detected for the safety of the patient and medical staff during examination. Motion profiling chair to safely move the patient by maintaining the head and back support to eliminate the need for patients to reposition as the chair back section raises or lowers. Low seat height minimum of 25 inches or less to safely transfer low mobility patients with minimal assistance (e.g. from wheelchair to procedure chair) without causing significant pain or discomfort and maximum of 40 inches or more to ensure effective examination and treatment of patients by medical staff of varying heights. Both hand buttons control and foot pedal control to safely position and reposition patients during examination, whichever will be closer and more ergonomic to the provider, while maintaining the safety of patients in preventing fall of patients during positioning/repositioning. Reset button present as one clearly marked button or present as a solitary button for repositioning of chair to its original upright position to allow for medical providers to continue to provide support to patients experiencing chronic pain during the transition/repositioning, eliminating potential for confusion caused by presence of multiple buttons, the assessment of which may divide providers attention. Chair Upholstery Top (including headrest and footrest) Adjustable headrest to ensure proper positioning and support of patients (whether in upright, prone, or supine positions) during examination to eliminate further injury. Adjustable footrest functions to provide the proper support to patients lower bodies and extremities (whether in upright, prone, or supine positions) while ensuring patients safety and sufficient comfort as tolerated by patients during providers examination. The footrest adjustability control through touch button(s) to accommodate patients as a preventative safety feature in diagnosis and treatment of bariatric patients. Upholstery surface easily able to be cleaned and sanitized per healthcare setting protocols. Chair Arms Upholstery surface easily able to be cleaned and sanitized per healthcare setting protocols. Installation 7.1. This requirement does not require an installation. Deliverables 8.1. Contractor is responsible for the delivery of the items listed in Product Description and Quantities (Section 3). Inspection and Acceptance Criteria 9.1. Products will only be accepted if they are determined to be of acceptable quality and working order, as determined by the VA Loma Linda Healthcare System Point of Contact. Warranty 10.1. Contractor shall provide standard warranty for the items listed in Section 3. Security Requirements 11.1. The contractor will not require any access to VA Information Technology systems nor unsupervised access to VALLHCS clinical areas. The information identified above is intended to be descriptive, not restrictive and to indicate the quality of the supplies/services that will be satisfactory. It is the responsibility of the interested source to demonstrate to the government that the interested parties can provide the supplies/services that fulfill the required specifications.
