- The purpose of HT001124R0042-0002 Amendment to RFP is to extend the proposal submission deadline from Thursday, 24 October 2024 at 1100 EST to Tuesday, 12 November 2024 at 1400 EST. This extension DOES NOT allow for submission of additional questions. Responses to the questions recieved will be issued via an amendment to the solicitation in the coming days. The purpose of HT001124R0042-0001 Amendment to RFP is to correct the due date for questions from 10 days from Block 8 of SF1449 to 10 days from Block 6 of SF1449.� See HT001124R0042-0001 with revised Addendum to FAR 52.212-1.� No additional questions will be accepted as the due date for all questions has past.� A list of questions and answers will be incorporated via an amendment to Solicitation HT001124R0042 in the coming days. The Defense Health Ageny is soliciting for Women's Health Quality Improvement support services.� This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and proposals are being requested. The Government intends to use Simplified Acquisition Procedures for evaluating proposals and awarding the resulting contract in accordance with FAR 13.106. This solicitation is a Total Small Business Set-Aside and restricted for small businesses only. The following documents have been attached for details and proposal submission: 1. Combined Synopsis 2. Attachment 1- Performance Work Statement� 3. Attachment 2- Pricing Sheet 4. Attachment 3- RFP HT001124R0042 (w/ provisions and clauses) 5. Attachment 4- Contract Data Requirements List (CDRL) Any questions in response to this notice shall be submitted in accordance with the instructions provided in the RFP.��
