D -- SYSTEM DESIGN FOR WEB-ENABLED VERSION OF ELECTRONIC PERSONNEL SECURITY QUESTIONNAIRE DUE 112497 POC Mike DeCloedt, Mailstop D0240 Defense Investigative Service (DIS) is conducting a market survey to evaluate options for obtaining system design, development, deployment, and maintenance support for a Web-enabled version of its Electronic Personnel Security Questionnaire (EPSQ). EPSQ is an automated data entry and validation system designed to allow users to quickly and easily enter the data required to conduct background investigations of incumbents or applicants for national security or public trust positions. The EPSQ corresponds to the Standard Forms 86 and 85a from the Office of Personnel Management (OPM) as well as the DIS Form 562. The EPSQ features subject and security officer editions for both contractors and military/government requesters. The EPSQ submits encrypted 86, 85a, and 562 information to DIS and provides receipts. Current versions were developed for DOS and Windows platform users. Currently fielded executable versions of the EPSQ software can be obtained from DIS on our Web site (www.dis.mil) or by contacting ourCustomer Service Center at 1-800-542-0237. DIS recommends respondents become familiar with the existing EPSQ to aid in preparing responses.
