MORTUARY SERVICES BLANKET PURCHASE AGREEMENT (BPA): This acquisition is
100% set-aside for small business. Interested offerors are requested to
submit a quotation on this requirement to 314 CONS/LGCV, ATTN: Maritza
Wine, 642 Thomas Avenue, Little Rock AFB, AR 72099-4971 or fax to
(501) 987-8119. Services will be provided on a call basis for the
period of five years; however, the BPA may be canceled by written
notification at an earlier time by Contractor or the Government.
Complete Part II, Schedule/Price List, with your prices to the
Government. Be sure to indicate the effective and expiration dates of
the price list. Complete Representation and Certifications. Return both
completed documents to the above indicated address or fax number. On a
separate sheet please indicate if you accept Government VISA credit
card. Award of BPA will be made based on overall rates provided to the
government. Contractor must be registered in the Central Contractor
Registration (CCR) (http://www.ccr2000.com) prior to receiving any
award. DESCRIPTION OF PRODUCT: Non-personal Services. The 314th
Contracting Squadron is acquiring mortuary services for Little Rock
AFB, Arkansas for handling the remains of deceased military personnel.
It is the policy of the Air Force to award contracts to fully
qualified professional funeral directors. The contractor shall provide
facilities, equipment, licensed personnel, motor vehicles, and furnish
professional services and requirements, supplies, ten death
certificates, caskets, urns, outer shipping containers, transportation,
and perform hygienic practices. The contractor shall comply with the
Armed Services requirements and specifications and all federal, state,
and local health laws, statutes and regulations. Solicitation is
